Phase I Management: Edema Secondary to Lymphatic Dysfunction
1. Complete decongestive therapy (CDT). a. Manual lymphatic drainage (MLD). • Massage (Vodder techniques, modifications by Asdonk, Leduc, and Fodi). Technique requires very low pressure effleurage strokes. • Emphasis is on decongesting proximal segments first at the right lymphatic duct (for right upper extremity involvement) and the thoracic duct (for left upper extremity, lower extremities, and the torso). • Compression using multilayered padding and short-stretch bandages. ○ Bandages have low resting and high working pressure. – Bandages preserve and advance changes associated with MLD and account for 50% of improvement in symptoms. ○ Treatments should be applied by certified specialists: certified lymphedema therapist (CLT). ○ Specialized education resources for MLD and CDT. – National Lymphedema Network: http://www.lymphnet.org. – Lymphology Association of North America: 315https://www.clt-lana.org.
Red Flag: Patients with significant or poorly controlled cardiopulmonary disease may become unstable if they are not able to tolerate the potential increase in plasma volume caused by lymphedema reduction.
b. Compression bandaging/garments. • Short-stretch compression bandages, worn 24 hours per day. • Bandages have low resting and high working pressure. • Bandages preserve and advance changes associated with MLD and may account for 50% of improvement in symptoms. • Must monitor distal symptoms and swelling for excessive pressure in off-the-shelf or customized garments.
Red Flag: Excessively high pressures will occlude superficial lymph capillaries and restrict fluid absorption.
c. Exercise. • Decongestive exercises work with bandages to compress lymph between the bandage and muscles to move fluid proximally. • Walking and cycling program. • Water-based programs: water aerobics, swimming. • Tai chi and balance activities. • ADL training.
Red Flag: Strenuous activities, jogging, ballistic movements, and rotational motions are contraindicated, as they are likely to exacerbate lymphedema.
• Be aware of lymph overload symptoms: discomfort, aching, or pain in proximal lymph areas, change in skin color. If any are present, discontinue activity.
d. Skin care: hygiene, skin care, nail care. • Protein rich fluids reduce local immune systems. • Bacterial infections are common: streptococcus, cellulitis. • Prevention of skin/nail problems. ○ Regular inspection: caregiver assistance if needed. ○ Regular cleansing. ○ Regular moisturizing: Maintain a pH of 5.0 on skin. ○ Protection: Appropriate footwear, use of appropriate socks.
e. Patient education (regarding skin care, garment wear, and other lymphedema self-management principles).
2. Contraindicated modalities. a. Modalities that cause vasodilation or increase lymph load (e.g., ice, heat, hydrotherapy, saunas, contrast bath, paraffin). b. Electrotherapeutic modalities greater than 30 Hz. |
