stretch receptor(muscle spindle, intrafusal fiber)

The knee jerk is the popularly known stretch reflex (involuntary kick of the lower leg) induced by tapping the knee with a rubber-headed hammer. The hammer strikes a tendon that inserts an extensor muscle in the front of the thigh into the lower leg. Tapping the tendon stretches the thigh muscle, which activates stretch receptors within the muscle called muscle spindles.

Each muscle spindle consists of sensory nerve endings wrapped around special muscle fibers called spindle fibers (also called intrafusal fibers). Stretching a spindle fiber initiates a volley of impulses in the sensory neuron (a I-a neuron) attached to it. The impulses travel along the sensory axon to the spinal cord where they form several kinds of synapses.

2. Some of the branches of the I-a axons synapse with inhibitory interneurons in the spinal cord. These, in turn, synapse with motor neurons leading back to the antagonistic muscle, a flexor in the back of the thigh. Muscle spindles are sensory receptors within the belly of a muscle, which primarily detect changes in the length of this muscle. They convey length information to the central nervous system via sensory neurons. This information can be processed by the brain to determine the position of body parts. 

근복내에는 근방추라는 감각수용기가 있어서 일차적으로 근육의 길이변화를 감지함. 근방추는 근육 길이정보를 감각신경을 통해 중추신경으로 전달함. 이 정보는 몸의 위치감각을 결정하는 뇌에 의해서 과정화됨.

The responses of muscle spindles to changes in length also play an important role in regulating the contraction of muscles, by activating motoneurons via the stretch reflex to resist muscle stretch.

Muscle spindles are found within the belly of muscles, embedded in extrafusal muscle fibers. Note that "fusus" is the Latin word for spindle. Muscle spindles are composed of 3-12 intrafusal muscle fibers, of which there are three types:

• dynamic nuclear bag fibers (bag₁ fibers)
• static nuclear bag fibers (bag₂ fibers)
• nuclear chain fibers and the axons of sensory neurons.

근방추는 근복내에서 발견되어 extrafusla muscle fiber 내에 둘러싸여 있음. 

근방추는 3~12개의 intrafusal muscle fiber를 포함함. 추내근 섬유(intrafusal fiber)은 3 타입이 있음.

핵주머니 섬유와 핵사슬 섬유 이해를 위한 1차구심 신경, 2차구심신경에 대한 이해

1. Type Ia sensory fiber

A type Ia sensory fiber, or a primary afferent fiber is a type of sensory fiber.^[1] It is a component of a muscle fiber's muscle spindle, which constantly monitors how fast a muscle stretch changes. In other words, it monitors the velocity of the stretch).

일차구심섬유는 감각신경임. 일차구심섬유는 근섬유의 근방추를 구성하여 근육이 얼마나 빨리 스트레치되는가라는 변화를 모니터함. 다른 말로 표현하면 스트레치의 속도를 모니터하는 역할. 

For the body to keep moving properly and with finesse, the nervous system has to have a constant input of sensory data coming from areas such as the muscles and joints. In order to receive a continuous stream of sensory data, the body has developed special sensory receptors called proprioceptors. Muscle spindles are a type of proprioceptor, and they are found inside the muscle itself. They lie parallel with the contractile fibers. This gives them the ability to monitor muscle length with precision.

인체가 정확함과 적절한 움직임을 유지하기 위하여 신경계는 관절과 근육으로부터의 감각데이터를 지속적으로 대뇌로 전달해야 함. 감각 데이터의 지속적인 연결을 받아들이기 위해서 인체는 고유수용감각이라고 불리는 특별한 감각 수용기가 발달해야 함. 근방추는 고유수용감각의 한가지 형태이고, 근방추는 근육자체 안에서 발견됨. 근방추는 근수축섬유와 평행하게 놓여져 있음. 근방추는 정확한 근육길이를 모니터하는 역할을 수행함. 

This change in length of the spindle is transduced (transformed into electric membrane potentials) by two types of sensory afferents, whose cell bodies are located in dorsal root ganglia located next to the spinal cord.

The two kinds of sensory fibers are different in respect to the kind of potentials they generate:

근방추 길이변화는 두가지 감각구심섬유에 의해서 신경발회되는데, 이것의 세포체는 척수에 dorsal root ganglia에 존재함. 

The first of the two main groups of stretch receptors wrapping the intrafusal fibers are the Ia fiber, which are the largest and fastest fibers, and they fire when the muscle is stretching. They are characterized by their rapid adaptation, because as soon as the muscle stops changing length, the Ia stop firing and adapt to the new length. Ia fibers essentially supply proprioceptive information about the rate of change of its respective muscle: the derivative of the muscle's length (or position).

위 두가지는 추내근 섬유를 둘러싼 스트레치 수용기에 두그룹임. 이 수용기는 크고 빠른 섬유임. 그것의 특징은 빠른 적응.

Type Ia fibers connect to both nuclear bag fibers and nuclear chain fibers. These connections are also called "annulospiral endings".

Type 1a 섬유는 핵주머니 섬유와 핵사슬 섬유에 연결되어 있음. 이러한 연결은 고리나선 신경종말이라고 함. "annulospiral endings"

In addition, the spindle also has a motor efferent innervation carried by gamma motor neurons, which is used by the nervous system to modify the spindle's sensitivity.

Ia afferents from the muscle spindle terminate on the proximal dendrites of motor neurons.

근방추로부터 올라오는 1a 구심섬유는 운동신경원의 proximal dendrite에 종지함. 

2. Type 2 sensory fiber

Type II sensory fiber (group Aβ) is a type of sensory fiber, the second of the two main groups of stretch receptors. They are non-adapting, meaning that even when there is no change in muscle length, they keep responding to stimuli. In the body, Type II fibers are the second most highly myelinated fibers.

type 2 구심섬유는 감각신경의 한 형태로, 스트레치 수용기의 두번째 .. 그것은 적응하지 않고 자극에 계속 반응함. type 2구심섬유는 잘 수초화된 섬유임. 

The muscle's instantaneous length, or position,is directly proportional to their firing rate. This information would indicate the position of one's leg once it has stopped moving. They do not respond to rate of length changes as do the Ia fibers.

근육의 즉각적인 길이, 위치는 그들의 발화율에 직접적인 비율을 가짐. 이러한 정보는 한다리로 서는 것을 말함.. 1a섬유처럼 길이변화비율ㅇ ㅔ반응하지 않음. 

Type II fibers connect to nuclear chain fibers, but not to nuclear bag fibers. These connections, referred to as "flower spray endings" due to their appearance, embed into the poles (ends) of the fibre. It is thought that the relative position of the equatorial regions of the spray when stretched determines the action potential output.

참고1. nuclear bag fiber

A nuclear bag fiber is a type of intrafusal muscle fiber that lies in the center of a muscle spindle. Each has a large number of nuclei concentrated in bags and they cause excitation of both the primary and secondary nerve fibers.

There are 2 kinds of bag fibers based upon contraction speed and motor innervation. 1) Bag2 fibers - are the largest. They have no striations in middle region and swell to enclose nuclei (hence their name) 2) Bag1 fibers - are smaller than bag2.  Both bag types extend beyond the spindle capsule.

추내근 섬유의 핵주머니 섬유는 근방추의 중앙에 있고, 각각의 주머니 내에는 많은 핵 숫자가 집중되어 있음. 핵주머니 섬유는 일차, 이차 신경섬유 양쪽을 다 흥분시킴. 핵주머니 섬유는 운동신경지배와 근수축 속도에 기초하여 2종류가 있음. 

bag2 섬유는 가장 큼.  중앙부위에 띠를 가지고 있지 않고, 핵을 둘러싸 둥글게 부어 있는 모양. 

bag1섬유는 bag2섬유보다 작음. 두개의 핵주머니 섬유는 근방추 캡슐을 넘어 확장되어 있음. 

참고2. nuclear chain fiber

A nuclear chain fiber is a specialized sensory organ contained within a muscle. Nuclear chain fibers are intrafusal fibers which, along with nuclear bag fibers, make up the muscle spindle responsible for the detection of changes in muscle length.

핵사슬섬유는  근육내에 있는 특수화된 감각기관임. 핵사슬섬유는 추내근 섬유로 핵주머니 섬유와 함께 근방추를 만들어 근육길이 변화를 감지함. 

There are 3–9 nuclear chain fibers per muscle spindle that are half the size of the nuclear bag fibers. Their nuclei are aligned in a chain and they excite the secondary nerve. They are static while the nuclear bag fibers are dynamic in comparison. The name "nuclear chain" refers to the structure of the central region of the fiber, where the sensory axons wrap around the intrafusal fibers.

하나의  근방추에는 3-9개의 핵사슬 섬유가 있고, 핵주머니 섬유 크기의 절반임. 핵사슬 섬유의 핵은 사슬안에 배열되어 있고, 2차 신경을 흥분함. 

핵주머니 섬유가 동적인 반면 핵사슬섬유는 정적임. 

The secondary nerve association involves an efferent and afferent pathway that measure the stress and strain placed on the muscle (usually the extrafusal fibers connected from the muscle portion to a bone). The afferent pathway resembles a spring wrapping around the nuclear chain fiber and connecting to one of its ends away from the bone. Again, depending on the stress and strain the muscles sustains, this afferent and efferent coordination will measure the "stretch of the spring" and communicate the results to the central nervous system.

2차 신경은 원심과 구심 전달로를 포함하여 근육의 긴장과 장력을 측정함. 

구심전달로는 핵사슬섬유 주위를 스프링 랩이 싸고 있는 것과 같은 모양. 

Also note that a similar structure attaching one end to muscle and the other end to a tendon is known as a Golgi tendon organ. However, Golgi tendon organs differ from nuclear chain and nuclear bag fibers in that they are considered in series rather than in parallel to the muscle fibers.

골지건기관으로 연결된 구조.. 골지건기관은 근육과 직렬로 연결되어 있오 근섬유와 평행하게 병렬로 연결된 핵사슬섬유와 핵주머니 섬유와는 구분됨. 

As intrafusal muscle fibers, nuclear chain fibers both send afferent innvervation and receive efferent innervation. The afferent innervation is via Group II and Ia neurons. These project to the nucleus proprius in the dorsal horn of the spinal cord. Efferent innervation is via static γ neurons. 

Stimulation of γ neurons causes the nuclear chain to shorten along with the extrafusal muscle fibers. This shortening allows the nuclear chain fiber to be sensitive to changes in length while its corresponding muscle is flexed.

추내근 섬유로서 핵사슬 섬유는 구심섬유로 보내고, 원심섬유로 받는 두가지 기능을 가짐. 구심섬유는 그룹 2, 1a 신경원을 통해서 신경지배되어 척수의 후각으로 전해짐. 원심섬유는 정적인 감마운동신경원을 경유함. 감마운동신경원의 자극은 핵사슬 섬유가 추외근 섬유와함께 짧아짐을 야기함. 이 짧아짐은 핵사슬섬유가 근육길이변화에 민감하고.....

Axons of gamma motoneurons also terminate in muscle spindles; they make synapses at either or both of the ends of the intrafusal muscle fibers and regulate the sensitivity of the sensory afferents, which are located in the non-contractile central (equatorial) region.^[1]

감마운동신경원의 축삭은 근방추내에 도달함. 그것은 추내근섬유의 끝이나 중간에서 시냅스로 연결하여 구심성 감각의 감수성을 조절함. 

Muscle spindles are encapsulated by connective tissue, and are aligned parallel to extrafusal muscle fibers, unlike Golgi tendon organs, which are oriented in series.

근방추는 결합조직으로 둘러싸여있고(fluid filled cavity), 직렬로 연결된 골지건기관과는 다르게 추외근 섬유와 평행배열되어 있음. 

The muscle spindle has both sensory and motor components.

• Primary and secondary sensory nerve fibers spiral around and terminate on the central portions of the intrafusal muscle fibers, providing the sensory component of the structure via stretch-sensitive ion-channels of the axons.

• In mammals including humans, the motor component is provided by up to a dozen gamma motoneurons and to a lesser extent by one or two beta motoneurons. Gamma and beta motoneurons are called fusimotor neurons, because they activate the intrafusal muscle fibers. Gamma motoneurons only innervate intrafusal muscle fibers, whereas beta motoneurons innervate both extrafusal and intrafusal muscle fibers and so are referred to as skeletofusimotor neurons.

Fusimotor neurons are classified as static or dynamic according to the type of intrafusal muscle fibers they innervate and their physiological effects on the responses of the Ia and II sensory neurons innervating the central, non-contractile part of the muscle spindle.

방추운동신경원은 정적 또는 동적 운동신경원을 분류됨. 이는 추내근섬유의 형태에 따라서 분류되고,  그것은 근방추의 비수축성 구조(중앙부분)에 신경지배하고 있는 Ia and Ib  감각신경의 반응에 생리학적 영향을 미침. 

• The static axons innervate the chain or bag₂ fibers. They increase the firing rate of Ia and II afferents at a given muscle length (see schematic of fusimotor action below).
• The dynamic axons innervate the bag₁ intrafusal muscle fibers. They increase the stretch-sensitivity of the Ia afferents by stiffening the bag₁ intrafusal fibers.

정적인 축삭은 chain 또는 bag2 섬유로 신경지배됨. 그것은 Ia and 2 구심신경의 활성화비율을 증가시켜 근육길이를 제공함. 

동적인 축삭은 bag1, 추내근섬유에 신경지배함. 그것은 bag1 추내근섬유 stiffening에 의해서 Ia 구심 신경의 스트레치 민감성을 증가시킴

The function of the gamma motoneurons is not to supplement the force of muscle contraction provided by the extrafusal fibers, but to modify the sensitivity of the muscle spindle sensory afferents to stretch. Upon release of acetylcholine by the active gamma motoneuron, the end portions of the intrafusal muscle fibers contract, thus elongating the non-contractile central portions (see "fusimotor action" schematic below). 

감마운동신경원의 기능은 근수축 힘의 제공이 아니라 스트레치에 대한  근방추 감각 구심을 조정하는 역할을 수행함. 감마운동신경원에 의해서 아테틸콜린이 분비되면, 추내근 섬유의 끝에서 수축함. 그래서 비수축성 중앙구조가 늘어남. 

This opens stretch-sensitive ion channels of the sensory endings, leading to an influx of sodium ions. This raises the resting potential of the endings, thereby increasing the probability of action potential firing, thus increasing the stretch-sensitivity of the muscle spindle afferents. 

감각신경 끝의 스트레치 민감성 이온채널이 열림으로 인해 sodium이온의 influx가 야기됨. 

How does the central nervous system control gamma fusimotor neurons? It has been difficult to record from gamma motoneurons during normal movement because they have very small axons. Several theories have been proposed, based on recordings from spindle afferents.

중추신경이 감마 방추운동신경원을 어떻게 조절하는가? 

움직임동안 감마운동신경원의 기록은 어려움. 왜냐하면 매우 작은 축삭이므로.. 구심 근방추를 기록하는 몇개의 이론이 제시됨 .

• 1) Alpha-gamma coactivation. Here it is posited that gamma motoneurons are activated in parallel with alpha motoneurons to maintain the firing of spindle afferents when the extrafusal muscles shorten.^[2]
• 2) Fusimotor set: gamma motoneurons are activated according to the novelty or difficulty of a task. Whereas static gamma motoneurons are continuously active during routine movements such as locomotion, dynamic gamma motoneurons tend to be activated more during difficult tasks, increasing Ia stretch-sensitivity.^[3]
• 3) Fusimotor template of intended movement. Static gamma activity is a "temporal template" of the expected shortening and lengthening of the receptor-bearing muscle. Dynamic gamma activity turns on and off abruptly, sensitizing spindle afferents to the onset of muscle lengthening and departures from the intended movement trajectory.^[4]

1) 알파-감마 상호활성화 

추외근 섬유가 짧아질때, 근방추의 활성이 유지되기 위해 감마운동신경원은 알파운동신경원과 평행하게 활성화됨. 

2) 방추운동신경 set 

감마운동신경원은 어려운 일이나 새로운 일을 수행할때 활성화됨. 반면에 정적 감마운동신경원은 보행과 같은 일상적인 움직임에 서 연속적으로 활성화됨. 동적인 감마운동신경원은 Ia 스트레치-민감성이 증가하는 어려운 일을 수행하는 동안 활성화되는 경향이 있음. 

3) 의도하는 움직임의 방추운동신경 본보기 

정적인 감마 활성은 수용기-bearing 근육의 예견된 짧아짐과 길어짐의 일시적인 본보기임. 동적인 감마 활성은 갑자기 켜졌다 꺼짐. 의도한 움직임으로부터 출발과 근육 늘어남의 시작 ... sensitizing spindle afferent....

stretch reflex

When a muscle is stretched, primary sensory fibers (Group Ia afferent neurons) of the muscle spindle respond to both changes in muscle length and velocity and transmit this activity to the spinal cord in the form of changes in the rate of action potentials. Likewise, secondary sensory fibers (Group II afferent neurons) respond to muscle length changes (but with a smaller velocity-sensitive component) and transmit this signal to the spinal cord. 

근육이 늘어날때, 근방추의 일차감각섬유(Ia 구심신경원)은 근육길이와 속도 변화에 반응하여 활동전위 비율변화의 형태를 척수로 전달함. 

마찬가지로 이차감각섬유(2 구심신경원)은 근육길이 변화에 반응하여(하지만 속도-민감성 부분에는 약간 반응)  신호를 척수로 전달함. 

The Ia afferent signals are transmitted monosynaptically to many alpha motor neurons of the receptor-bearing muscle. The reflexly evoked activity in the alpha motoneurons is then transmitted via their efferent axons to the extrafusal fibers of the muscle, which generate force and thereby resist the stretch. The Ia afferent signal is also transmitted polysynaptically through interneurons (Renshaw cells) which inhibit alpha motoneurons of antagonist muscles, causing them to relax.

1a 구심신호는 수용기-bearing 근육의 많은 알파운동신경원에 monosynaptically하게 신호전달함. 알파운동신경원에 반사적으로 발생한 활성은 추외근섬유에 원심성 축삭을 경유하여 전달함. 이것은 힘을 생성하고 스트레치에 저항함. 1a  구심신호는 polysynaptically하게 렌쇼 세포(사이신경원)로 전달하여 길항근의 알파운동신경원을 억제하여 근육을 이완함. 

참고) Interneuron

An interneuron (also called relay neuron, association neuron, connector neuron or local circuit neuron) is a neuron that forms a connection between other neurons. Interneurons are neither motor nor sensory. The term is also applied to brain and spinal cord neurons whose axons connect only with nearby neurons, to distinguish them from "projection" neurons, whose axons (projection fibers) project to more distant regions of the brain or spinal cord.

After stroke or spinal cord injury in humans, spastic hypertonus (spastic paresis or spastic paralysis) often develops, whereby the stretch reflex in flexor muscles of the arms and extensor muscles of the legs is overly sensitive. This results in abnormal postures, stiffness and contractures. Hypertonus (Hypertonia) may be the result of over-sensitivity of alpha motoneurons and interneurons to the Ia and II afferent signals.^[5]

중풍이나 척수신경손상후에 강직성 근마비가 진행되는데, 팔의 굴곡근과 다리의 신전근에서 스트레치 반사가 과도하게 발생함. 그 결과 비정상적인 자세, 뻣뻣함과 구축을 유발함. 근긴장증은 알파운동신경원과 1a 와 2 구심 신호에 사이신경원이 과도하게 민감한 결과임. 

PNF stretching, or proprioceptive neuromuscular facilitation, is a method of flexibility training that can reduce hypertonus, allowing muscles to relax and lengthen.

 PNF 스트레칭은 유연성 트레이닝 방법을 이용하여 과도한 근긴장을 줄여줄 수 있어 근육의 이완과 늘어남을 허용하게 함.
