4.흉추(Thoracic spine)

(1)concept of treats

thoracic spine은 rib과 붙어있으므로 그 mobility가 매우 적다. 또한 prone position에서 treats할 경우 rib에 over-loading power가 가해져서 rib이 injury되는 경우도 있는 만큼 treats에 caution을 해야 한다.

 

(2)supine position thoracic spine extention technique

❶indication

thoracic spine의 general인 extention

❷concept

환자는 treats table에 앉아서 손은 깍지끼어 머리뒤로 돌리고 팔꿈치를 벌린다.

치료사는 환자의 팔사이로 손을 집어넣어 손바닥으로 환자의 아래쪽 thoracic post region을 잡는다.

치료사는 환자의 등뒤로 한쪽무릎을 굽혀 환자가 thoracic spine을 치료사의 thigh에 기대게 하고 치료한다. 환자를 약간 위로 끌어 올리며 가슴을 벌리듯 환자의 양팔과 흄부를 누른다.

❹치료사의 접촉점(contact point)

both hand,finger

❺치료방향(line of correction)

caudal에서 cephalic쪽으로

anterior에서 posterior으로

❻contra indication

shoulder에 이상이 있거나 cervical spine에 problem이 있는 경우

 

(3)sitting position thoracic spine extension technique

❶indication

thoracic spine의 general인 extension 및 subluxation adjustment technique

❷concept

환자를 앉힌뒤 both hand를 posterior cervical spine에로 깍지 끼게 한다. 치료사는 무릎을 굽히고 treats table에 쪼그려 앉아 patient의 thoracic spine을 치료사의 무릎으로 support한다. 치료사의 both hand는 patient의 깍지낀 both arm사이로 넣어 patient의 upper arm을 잡는다.

patient에게 deep breathing을 하게 하고 expiration이 끝날 때 both arm을 당겨서 치료한다.

special part를 치료하고자 할 때에는 치료사는 무릎을 치료하고자 하는 thoracic spine의 한쪽, 혹은 양쪽 transverse process에 대고 앞서와 같은 방법으로 치료한다.

치료시-치료하고자 하는 thoracic spine의 transverse process

❹치료사의 contact point

knee

❺caution point

knee으로 contact하는 환자의 thoracic spine에 pain을 느낄 수 있으므로 knee에 towel을 대고 contact하는 것이 좋다.

 

 

(4)upper thoracic spine extension technique

❶indication

upper thoracic spine의 fixation

❷concept

patient는 어깨넓이 정도로 다리를 벌리고 양손을 목뒤로 깍지끼고 선다. 치료사는 환자의 뒤에 서서 한쪽다리를 patirent의 다리 사이로 넣고 patirent의 axillar 아래로 손을 넣어 환자의 상박을 양손으로 가볍게 잡는다. 환자에게 뒤로 몸을 눕혀 치료사의 가슴에 등을 기대게 한다.

patirent에게 deep breathing을 하게하고 patirent의 expiration시에 patirent의 상박을 위로 들어올리듯 하며 환자의 등이 신전될 수 있도록 patirent의 shoulder joint를 internal rotation 시켜준다. 이 때 치료사의 가슴으로 환자의 등을 받쳐 들어올리듯 하면 좋다.

(5)prone position both hand pisiform thoracic spine transverse process adjustment technique 1

❶indication

thoracic spine의 rotation subluxation 및 posterior subluxation

❷concept

환자의 prone position에서 치료하고자 하는 spine의 양쪽 transverse process에 치료사는 both hand의 pisiform을 contact시킨다.

치료하고자하는 방향으로 회전력(torque power)을 주며  transverse process를 누르며 moment adjust한다.

그림과 같이 양손을 교차시켜서 치료하거나 아니면 그냥 치료해도 좋다.

양손의 pisiform

❺치료방향(line of correction)

posterior에서 anterior

회전력-추골의 상태에 따라

그림에서 가운데의 흉추는 좌상변위 되어있다. 이 때 양측의 횡돌기를 두상골부로 접촉하여 화살표방향으로 회전을 시켜 주며 순간교정한다.
