수술명
1 Incision of vessel (embolectomy or thrombectomy), unspecified

수술소견

타병원에서 femoropopliteal bypass with ringed graft 8mm,Lt. 시행된 환자로 claudication을 주소로 내원함.
우측은 saphenous vein을 이용한 F-P 상태이며 distal anastomosis site의 stenosis있으나 ABI 1.01.로 1개월뒤 PTA결정 예정임.

좌측 Graft 전체와 popliteal artery 3cm(P2 lesion)까지 thrombosis가 있었고 common femoral artery mid part까지 thrombosis 있는 상태였음.

수술절차

1.Incision and adhesinolysis
2.CFA,DFA,Proximal graft snaring
3.graft opening 하여 deep femoral artery로 fogaty 3Fr 진입시도하였으나 진입되지 않음..
4.C-arm을 이용하여 graft thrombectomy cath로 graft distal 부분부터 thrombectomy 시행하고 fogaty 3,4Fr.를 이용하여 naive popliteal arteyr thrombectomy시행함.
5.diagnotic cath와 C-ARM으로 Angio시행하였고 popliteal artery의 diffuse stenosis와 remnant thrombosis있어
  distal anastomosis site opening하여 thrombectomy 시행함.
6.yamada balloon 진입 시도하였으나 lesion stenosis 심해 distal popliteal artery 까지 통과 되지 않아 rival ballon을 이용하여 preballooning 시행후 zilver PTX 6mmx60mm stent로 PTA 시행함.
7.이후 Wound closure후 발의 cyanosis acute하게 발생하여 wound reopenig하여 3Fr.4Fr. thrombectomy cath로 thrombectomy 시행하였고 common femoral artery의 stenosis있어 이를 제거하고 8mm interguard graft로 기존 graft에 end to end pattern으로 anastomosis 시행함.
8.이후 angiography상 flow 원할하여 수술 종료함.
