[두개천골리듬(Craniosacral Rhythm)]
두개천골리듬은 두개천골치료법(Craniosacral Therapy)에서 기인한 용어입니다. 이 두개천골 치료법(CST)은
수기의학(manipulative medicine)에서 빠르게 성장하고 있는 분야 중 하나입니다.
두개천골리듬은 20세기 초 써덜랜드(Sutherland, 1939)에 의해 개발되어 두개골요법(Craniopathy, Cottam, 1956), 천골후두골테크닉(SOT, Sacrooccipital Technique, Dejanette 1975/1978) 등의 유사한
두개골을 중심으로 하는 치료법을 발전시킨 학파들에서 그 이론적 기반으로 사용하고 있습니다.
두개천골리듬을 두개골만을 중심으로 설명할 때는 '두개골의 율동적 임펄스(CRI, Cranial Rhythmic Impulse)'라고
합니다.

'두개골의 율동적 임펄스(CRI, Cranial Rhythmic Impulse)'를 만들어내는 원인이
무엇인지에 대한 뚜렷한 설명은 아직까지 없지만 CRI의 존재에 대해서는

이 CRI는 두가지의 움직임으로 구성되어 있습니다.
하나가 Flexion 이고 다른 하나는 Extension 이라는 움직임입니다.
여기서 Flexion과 Extension은 골 운동을 설명할 때 쓰는 굴곡(flexion), 신전(extension) 과는
구별해서 씁니다.

Flexion 이란 움직임은 두개골과 신체의 전후면(AP, Anterior-Posterior)이 감소하는 움직임입니다.
전후면(anteroposterior dimention)의 감소는 가로축(width)의 길이를 늘리게 됩니다.
(여기서 width는 사람이 앙와위(supine) 자세로 누웠을 때 양쪽 귀를 연결한 선이라고
생각하시면 됩니다. ), Extension은 이와 반대되는 움직임입니다.

Flexion시 SBS에서의 후두골(Occipital bone)과 접형골(Sphenoid bone)의 움직임

이 두개천골리듬은 그 주기가 대략적으로 1분에 4~10회(또는 그 이상) 정도 되는 것으로
알려져 있습니다.(Normal CRI에 대해서는 여전히 의견이 분분합니다. 아래 표는 CRI에 대해 측정한
사람들과 리듬, 방법에 대한 것입니다.)

아래는 CVH(Cranial Vault Hold)라는 테크닉입니다.
CRI를 검사하는 기본적인 테크닉입니다.
테이블에 환자를 앙와위로 눕히고 치료자는 테이블의 머리맡에서 팔이 피곤해지는 것을
예방하기 위해 전완(forearm)부위를 테이블 위에 놓습니다.
그리고 A, B, C, D 위치에 엄지손가락을 제외한 네 손가락이 닿도록 위치를 잡습니다.
이 때 치료사의 집게손가락이 접형골의 대익(the greater wings of the sphenoid)에 닿을 수 있도록 하고
손바닥 전체가 두개골을 감싸듯이 모양을 취합니다.
이 때 가운데 손가락은 측두골의 관골돌기(zygomatic processes)에 약지는 유양돌기(mastoid process)에 닿게하고
새끼손가락은 후두골의 비늘(린, 鱗) 부위(the squamous portion of the occiput)에 닿게합니다.
엄지 손가락은 서로 교차하거나 가볍게 접촉시키되 머리 부위에 닿지 않게 합니다.
CRI를 성공적으로 촉진하기 위해서는 편안한 마음으로 정신을 집중해야 합니다. 릴렉스 ~~~
접촉 시 가하는 압력은 약 5 그램 정도의 느슨한 압력입니다.
눈은 감고 어깨와 손은 긴장하지 말아야 합니다.
먼저 2~3분 동안 심장혈관과 호흡 그리고 다른 종류의 다양한 박동감을 감지해 봅니다.
그리고 나서 CRI를 감지해봅니다.
두개천골리듬이 원활하게 되면 다음과 같은
긍정적인 효과가 나타납니다.
1) 스트레스가 감소되고(It reduces stress)
2) 긴장된 신경계가 안정되며(It calms the nervous system)
3) 혈압이 감소하고(It lowers blood pressure)
4) 올라갔던 열이 내려가고(It reverses a fever)
5) 면역계의 효율성이 증가하고(It increases immune system effectiveness)
6) 두통을 감소시키고(It reduces headaches)
7) 관절염 치유도 일어나게 됩니다.(It treats arthritis)

The objective of Cranial Vault Hold Technique is to assess
the primary respiratory mechanism as it manifests itself in the
cranium and the degree of participation of each bone in the general motion of the cranium.
Palpation of CRI
a. Extension/internal rotation: coronal diameter narrows, anteroposterior diameter increase,
height increase.
b. Flexion/extenal rotation: coronal diameter widens, anteroposterior diameter decrease,
height decrease.
Notes the amplitude, rate, and regularity of the CRI.
Notes which bones, if any, have an altered amplitude, rate regularity.
The therapist may instruct the patient to stop breathing to further distinguish
the rhythmic sensations that occur in the CRI. The therapist can also
have the patient inhale and exhale fully to
incerase the amplitude of the CRI, which can make it easier to feel.

Upledger noticed a pronounced postural habitus that correlated
with two kinds of disturbance to the craniosacral rhythm
- towards the 'flexion' or 'extension' phase(at the sphenobasilar junction)
He observed two polar postural types:
(1) the 'flexion lesion' type, who are externally rotated at their extremities,
have a broad cranium and a 'waddling quality' to their gait; and
(2) the 'extension lesion' type, who have internally rotated extremities and
a narrower cranial vault.
Craniosacral therapists of the Upledger school call them flexion or extension types.
This model, however, has been considerably augmented and extended
within the Rolfing community.
