Trichloroacetic acid (TCA) is synthetically produced from acetic acid and chlorine. Despite what some may like you to believe, TCA is one of the least expensive chemicals for chemical exfoliation and peels being less expensive than glycolic acid. The extreme price mark up for TCA chemical peels has nothing to do with the grade or quality of the TCA, but instead has everything to do with a consumers' desire to have a beautiful complexion, how quick results are seen from the use of TCA, and the sellers desire to make a small fortune from the sale of TCA. Some say that if you see TCA for $20 bottle stay away because it is cheap industrial grade TCA, but guess what, this is only said to protect their high dollar retail price and profit margin because all TCA is cheap and since 1990 when TCA was no longer included in the United States Pharmacopoeia, TCA officially lost its' medical grade status which means all TCA is industrial grade because TCA is originally an industrial chemical. Regardless of this true and verifiable fact, distributors can still label their TCA as medical grade as long as the purity of the TCA is the same purity or higher of the TCA that was once classified as USP medical grade back in the 80s, this is a rule for the USP.
What is TCA- TCA is a non-toxic chemical which has been used to perform skin peels for over 20 years. It is a relative of vinegar (acetic acid.) When TCA is applied to the skin, it causes the top layers of cells to dry up and peel off over a period of several days to one week. When the old skin is peeled off, it exposes a new layer of undamaged skin, which has a smoother texture and more even color, therefore TCA peels are great for evening out skin tone and getting rid of imperfections, and unlike AHA peels, TCA can even get rid of finer wrinkles.
The resurfacing depth achieved with TCA ranges from superficial to deep depending on the acid concentration. It is commonly used as pure TCA in concentrations up to 20% for superficial peels, 25%-35% for medium depth peels, 40%-50% for deep peels. A face peel over 35% should always be applied by a doctor. Applying 35% or higher to other parts of the body is fine.
Superficial Peels 8%-20% Also known as lunch time peels, these work by speeding up the renewal process on the top layer of skin, the epidermis. Superficial peels provide instant gratification in the form of a luminous complexion with little, if any downtime aside from a few days of tight, dry skin. Superficial peels treat dry skin, soften fine lines, fade mild hyperpigmentation and acne spots, and tend to be the safer choice for most skin types. With that being said, a series of superficial chemical peels will be needed to achieve optimal results and the results will not last as long as a medium/medium deep peel.
Medium Depth Peels 25%-35% Medium depth peels penetrate more deeply getting to the border or the dermis to remove the top few layers of skin where the blemishes, sun spots, and other imperfections lives. Medium depth peels create a deliberate wound, like a sunburn, that allows the new skin to come thru rejuvenating the complexion. Medium strength peels do everything that the superficial peel does, but is more effective and because it goes deeper into the dermis, it is more effective at stimulating collagen to tighten the skin. Medium depth peels requires downtime, at least a week.
Deep Peels 40%-100% TCA at this strength should never be applied to the face. Purchasing TCA in strengths of 50% or higher should be diluted to lower strength if a face peel is desired. This higher strength can be applied to other parts of the body for legs, hands, dark knees & elbows, stretch mark fading, and tattoo fading. Always start at a 40% strength to test skin tolerance before moving on to a higher strength.
TCA Peel Candidates - Fairer-skinned persons are considered to be the best candidates for chemical peels. Darker-skinned persons can have successful tca chemical peel procedures, however the TCA peel strength should always remain with the superficial peel range. Conditions for effective results from TCA peels generally include, but are not limited to:
Skin that has developed brown spots;
Wrinkling caused by the onset of aging;
Sun damage
Complexion that has become dull.
Skin Preparation- For optimal results it is best to prime the skin for the TCA peel. Performing a lactic acid or glycolic acid peel at least 2x within a 2 week time frame prior to the TCA peel will do this. Using an alpha hydroxy acid cream daily for 2 weeks prior to the TCA peel will also do this.
