Blood flow restriction training (also abbreviatedBFR training[1]) or occlusion training is an exercise approach whereby resistance exercise or aerobic exercise is performed whilst an occlusion cuff is applied to proximal aspect of the muscle[2]. In this novel training method, limb blood flow is restricted via the occlusion cuff throughout the contraction cycle and rest period. This results in partial restriction of arterial inflow to muscle, but, most significantly restricts venous outflow from the muscle[3]. Given the light-load nature and strengthening capacity of BFR training, it can provide an effective clinical rehabilitation stimulus without the high levels of joint stress and cardiovascular risk associated with heavy-load training[4]
Many practitioners use The Occlusion Cuff as well as BFR protocols that do not match usage within published literature[5]. The current approaches that focus on applying BFR during exercise consist of automatic pneumatic tourniquet systems or handheld inflatable device called The Occlusion Cuff. Research demonstrating the influence of thigh circumference and cuff width [6] on occlusion pressure has accentuated a likely need for an individualised approach to BFR, particularly with regard to the setting of the restriction pressure. More recently, a technique to calculate and prescribe the occlusive stimulus as a percentage of total limb occlusion pressure is just one example of efforts to account for the above factors and provide an individualised approach to prescribing BFR training that is relatively quick and inexpensive. While the relationship between BFR pressure and the underlying tissue compression during exercise is not yet fully understood, BFR training using 40%–80% of limb occlusion pressure is safe and effective when supervised by experienced practitioners [7] ; therefore, lower pressures may provide less risk without the need for higher pressure.
Historically, heavy exercise loads of approximately 70% of an individual's one repetition maximum (1RM) have been deemed necessary to elicit muscle hypertrophy and strength gains[8]. In recent years, research has demonstrated that augmentation of low-load resistance training with blood flow restriction (LL-BFR) to the active musculature can produce significant hypertrophy and strength gains [9][10], using loads as low as 30% 1RM[11]. BFR training has been found to yield hypertrophy responses comparable to that observed with heavy-load resistance training [12]. Recently, it has also been indicated that BFR training induces beneficial changes in tendon structure and tendon stiffness [13].
When using belts and lifting straps for tourniquet, the amount of pressure on the vasculature cannot be controlled and there are reports of rhabdomyolysis cases due to VOT.[14] A proponent of the method, Dr. Yoshiaki Sato, risked his life in 1967 after uncontrolled sessions of VOT, leading him to focus more on control and adjustment of the pressure applied on the vasculature.

근육을 키우는 동작을 하는 동안에, 이 압박밴드가 혈관을 압박해 피를 막는 게 근육을 더 많이 만들어주고 심지어는 물리 치료 효과도 있다는 것이다.
‘혈류 제한 요법(BFR)’으로 불리는 이 방법은, 가벼운 운동을 할 때나 심지어 운동을 하지 않을 때에도 근육 유지에 효과가 있다고 한다. 미국 미시시피대 보건체육학과 제러미 로에네케 교수는 “운동을 하지 않을 때에도 혈류를 제한하는 방법은 근육의 크기와 강도를 유지하는데 도움을 주며, 재활에도 잠재적으로 효과가 있다”고 말했다.
BFR를 통한 근육 강화 원리는 이렇다. 근력 운동을 할 때 무거운 덤벨을 들면, 근육 자체에 균열이 발생한다. 그리고 쉬면서 근육은 이 부분을 회복한다. 용해된 섬유 조직은 뭉치면서 새로운 근육 조직 가닥을 만들어낸다. 근력 운동을 하면, 이 과정을 되풀이하면서 근육이 자라고 단단해지는 것이다. 이때 BFR 방식을 이용하면, 정맥으로 가는 혈류를 막아 근육이 자라는 과정을 더 빠르고 효과적으로 돕는다고, 데일리 메일은 전했다.
헬스 트레이너인 리 브로건은 “그저 멋 내기용(用)이 아니라, 과학적으로 효과가 입증된 방법”이라고 말했다. 그는 “정맥으로 흐르는 혈류를 압박하면서 동맥을 열어두면, 근육 내에 피가 모이게 된다. 이는 근육을 팽창시키고 펌프질하는 효과를 낸다. 실제로 혈류를 압박하면 근육이 팽창한 모양을 즉시 육안으로 확인할 수 있다”며 “이 과정에서 근육 성장에 필요한 젖산과 신진대사 물질이 분비된다”고 말했다. 그는 BFR 방식이 또 다치지 않고 근육 성장 효과를 볼 수 있는 방법이라며 추천했다.
이 밴드는 재활 치료에도 효과적이다고. 일본 내에서 ‘캇츠(KAATSU)’로 불리는 이 밴드는, 처음 ‘혈류 제한 요법’을 개발한 요시아키 사토가 붙인 이름이다. 그는 스키를 타던 중 발목이 부러져 물리치료를 받으면서 이 원리의 효과를 입증해 밴드를 발명하게 됐다. 그는 치료 중에 근육 수축 운동을 하며 이 밴드로 다리를 묶어 예상보다 빨리 6주 만에 발목 치료를 마쳤다. 그는 팔다리가 부러질 때 근육이 줄어드는 걸 방지하고 근육 파열 등 부상 통증도 완화한다고 밝혔다.
‘브리티시 메디컬 저널’도 경량(輕量) 운동을 할 때 혈류 압박을 두 배 높이면 근골격 부상을 입은 환자의 근육 강화가 훨씬 빠르다는 연구 논문이 게재됐다. 하지만, BFR은 체중 감량과는 무관하다. 그저 동일한 체중 상태에서 근육을 효과적으로 만드는 기법일 뿐이라고 한다.
