TO ASSIST INFANTS AND ADULTS IN SLEEP

ABSTRACT
Repetitive sound stimulation has been proven to help soothe even irritable or colicky infants and to reduce crying. It has also been proven to help both infants and adults to relax and fall asleep faster and to achieve deeper levels of sleep than without sound stimulation. Few outside the academic community are aware of this , and fewer still take advantage of this knowledge in helping to soothe infants and to lull adults to sleep. The author reviews 70 years of research with the intent of selecting and presenting the most soothing sounds in a simple and effective device for consumer use. The sounds determined to be best for infants were “womb”, “heartbeat” with white noise background and “lullaby”. The sounds selected for people of all ages were “surf”, “rain” and “brook”.

INTRODUCTION
Throughout history, in all culture, people have held babies sounds would soothe a child and help it to stop crying and to fall asleep. Additionally, it seems that mankind has always found water sounds, such as a mountain stream, ocean surf or rainfall, to be very relaxing and sleep inducing. Serious scientific research into this subject began more than 50 years ago and has proven that these sounds are very effective at helping lower stress levels, to mask irritating noise and to help people fall asleep – and to sleep better. This paper will provide a brief summary of such research; although, it is not meant to be an all encompassing encyclopedia of such research. Special emphasis is given to newborns and very young children, including some information on colic and how sound can help soothe many colicky infants.

1920s to 1950s: EARLY RESEARCH

Some of the earliest formal research concerning repetitive stimulation to assist in sleep was conducted by the world famous Pavlov. In 1922, he observed that his dogs fell asleep after continuous and monotonous stimulation during conditioning experiments. Pavlov called this effect “internal inhibition” (Pavlov, 1927, 1928)

In 1934, the first systematic study of the effects of continuously presented sound on infant behavior was conducted by Weiss. She used pure musical tones generated by an oscillator and concluded that all continuous tones had a significant pacifying effect, and that lower tones were more effective than higher tones. (Weiss, 1934) Stubbs(1934) drew essentially the same conclusions in a separate study.

In 1942, Lovell and Morgan conducted extensive research that led them to conclude “that monotonous sound simulation is usually followed by relaxation, that respiration rate is one of possible many physiological response affected by the stimulation, and that this response probably aids in the relaxation.” (Lovell & Morgan, 1942, p. 448) They also found that “the reaction of adults to the sounder was found similar to that of children in that both groups relaxed.” (Lovell & Morgan, 1942, p. 449)

1960s to 1970s: FINDING THE MOST SOOTHING SOUNDS

There was a tremendous amount of research conducted in the early 1960s studying the relationship of sound and sleep. By 1963, there was enough evidence concerning repetitious sound to encourage a leading sleep researcher to state: “Production of monotonous sounds, as a soporific agent, is, of course, and ancient procedure.” (Kleitman, 1963, p. 311) Following is brief summary of some of the more significant findings.

Following studies by other researchers with chimpanzees, Salk proved in several experiments that heartbeat sounds helped soothe neonates. (Salk 1960, 1961, 1962). Shortly thereafter Brackbill, Adams, Crowell and Gray (1966) demonstrated that a recorded lullaby was even more effective than a recorded heartbeat. At about the same time, Birns, Blank, Brigder and Escalon (1965) were updating the research with musical tones conducted by Weiss (1934) and Dtubbs (1934) and confirmed that low tones work better than high tones. Most of this research measured reduced crying awake time, and/or how fast babies stopped crying with the use of monotonous sound versus no sound. Brackbill (1971) reported that continuous sound stimulation “has little effect on the amount of time spent awake and quiet, but a very large (reducing) effect on the amount of time spent awake and crying.”(Brackbill, 1971, p21).

In 1959, Wolff began experimenting with white noise and concluded that continuous white noise had quieting or sleeping promoting effect on young infants. (Wolff, 1959) White noise occurs in nature as wind and water sounds, especially rain, waterfall and ocean surf. In 1967 Wolff reported that, in neonates, “irregular sleep” or active sleep, could readily be converted into “regular sleep” or quiet sleep by application of continuous white noise. (Wolff 1967).

By the late 1960s and early 1970s, most researchers switched to the use of white noise in their experiments, and began to explain, not just that continuous and monotonous sound worked to help infants and adults relax and fall asleep, but how and why. This research is beyond the scope of this paper; however, the reader may refer to the bibliography to read more about sleep states, brain activity during sleep, neuronal discharges during sleep, REM and no-REM sleep states, and the neurophysiologic organization of sleep states and their development from infancy to childhood. Following alphabetically are some of the researchers: Aserinsky & Kleitman (1955), Compos & Brackbill (1973), Dittirichova (1996), Parmalee. Jr. Wenner, Alkiyama, Schultz (1967). Parmalee, Jr. and Stern (1972), Prechtl, Akiyama, Zinkin and Grant (1968), Roffwarg, Muzio and Dement (1966) and Schaefer (1975).

The development of ultrasound technology for monitoring fetal activity ultimately led to the hypothesis that newborns might be soothed best by the sounds that the baby heard before it was born. Proof came in 1975; when a group of researchers in Japan reported that over 80 percent of babies fell asleep in under 5 minutes when played recordings of intrauterine sounds, especially the rhythmic “whoosh” of the maternal blood flow to the placenta and the baby. (Murooka, Tsutoma, Tsuyoshi, Yusuo, Mitsukazu and Nobuhiro, 1975), these results place womb sounds among the very best at soothing babies.

VOLUME, AGE AND OTHER MODALITIES OF STIMULATION

It is important to note that research in the 1960s and 1970s also proved that, to be effective for newborns, any of these sounds had to be fairly loud by adult standards. Specifically, a repetitive sound played at 85 db soothed more effectively than at lower levels; and , at below 60 db , the sounds were ineffective for most infants. (Ashton, 1971; Brackbill, et. al,. 1966; Brackbill, 1970, 1971, 1973; Keen, 1964; Murray & Campbell, 1971; Salk, 1962; Spencer, Moran, Lee and Talbert, 1989; Wolff, Matsymiya, Abroms, Van Velzer and Lombroso, 1974). It is interesting to note that, according to at least one source, the average acoustic intensity inside the human uterus is 85db. (Grimwade, Walker and Wood, 1970.) Sound intensity of 85 db is like a loud vacuum cleaner or hair dryer. Adults might find this volume level uncomfortable, but it should not be harmful, A recent study in London stated: “ it seems unlikely that use of this device (white noise generator) will deprive infants of feeds, should they be required. There is further evidence to suggest that white noise, used for induction of sleep in babies, does not suppress intrinsic stimuli.” (Spencer, et. al.. 1989 p.136)

Another important discovery is that Kopp (1970) found that electronic white noise at 85 db soothed crying very effectively up to two months of age, but became less effective at three months. At three months, it is believed that babies start to distinguish “social sounds”, and that loud “non-social sounds” like white noise begin to aggravate rater than soothe. At this time, it is more effective to begin turning down the volume, and it may be more effective to switch to a lullaby, nature sound or other repetitive “social” sound.

Finally, research in the 1960s and early 1970s indicated that monotonous stimulation of the senses other than hearing can help babies stop crying. These other areas investigated were touch (swaddling), motion (rocking), light and temperature. According to Brackbill and Fitzgerald (1969, p.177): “infants are less aroused in a noisy environment than in a quiet one, less aroused under high illumination than in darkness, less aroused when swaddled or clothed than when naked, and less aroused when being jiggled than when lying quietly in their cribs.” Furthermore, they found that using two of these modalities was more effective than one; three were more effective than two; and, four were more effective than three. (Brackbill & Fizgerald, 1969).

In recent years, most scientists acknowledge the effectiveness of repetitive sounds to help healthy babies and adult to fall asleep faster and to achieve deeper levels of sleep. Most recent research has been conducted to determine if these sounds can help in special health situations: specifically, in helping colicky babies, severely stressed adults, and insomniac elderly patients.

COLIC
Colic, for purposes of paper, is defined by Keller, Ubozak and Rissau (1990, p.72) of the university of Osnabruuck, in Germany: “onset during the first week of life, maximum at around the sixth week, consisting of fussing or crying for at least three hours a day, and occurring on more than three days in more than three consecutive weeks; easing off at about three to four months.” It is estimated that 10% (Brazelton, 1990) to 40 % of all infants suffer from colic (Covington, Cronenwett and Lveland-cherry, 1991; Fuller , Keefe and Curtain, 1994: Hide & Guyer, 1982; IlIingsworth, 1954; Larson & Ayloon,1990; Wahlberg, 1990). For purposes of this paper, colic is assumed not to have a health related cause. If there are gastrointestinal problems or allergic reactions to milk or formula, etc. they must be appropriately handled by the baby’s pediatrician. Several researchers indicated that “Colicky or irritable” babies cry more stridently than “ normal” babies. Researchers used terminology like higher in jitter, shimmer, proportion of noise and tenseness, (fuller, et. al. 1994) and more variable pitch, more turbulent or dysphonic, (Lester, Boukydis, Garcia-Coll and Hole, 1992) Others like Barr(1993) argued that once clinical disease has been ruled out, it is pointless to argue whether the infant has something called “colic”, or simply cries more than most. For solutions, many researchers select the soothing stimulation of repetitious sound, combined with swaddling, and carrying or rocking – in conjunction with parental education concerning colic and infant behavior. (Barr, 1990, 1993; Brazelton, 1990; Carey, 1990; Keller, et. al. 1990; Larson & Alyylon, 1990; Pinyerd 1992; Sloman, Bellinger and Krentzel, 1990).

STRESS REDUCTION IN ADULTS AND HELP FOR INSOMNIA

A good example of recent research confirming the ability of repetitive sound to help soothe severely stressed adults was conducted at a leading coronary hospital by J.W. Williamson. Sleep disturbance and high stress levels are common with hospitalization, especially with intensive care patients. Dr. Williamson tested the use of ocean sounds, played on Sound Conditioner with 60 coronary artery bypass graft patients immediately after transfer from the Intensive Care Unit. Results indicated that subjects had significantly deeper and better quality of sleep, fewer nighttime awakenings, quicker return to sleep, and better overall sleep scores than controls ( Williamson, 1992).

While most of the research on insomnia in recent years has been for testing various medications, it is refreshing to note that repetitive sound can provide safe, drug free therapy. Recent research in Australia again confirmed this hypothesis. Continuous auditory stimulation was tested over a 4 week time span on 58 insomniacs, aged 18 to 58. Results indicated that there was significant improvement in sleep onset, latency, duration of wakefulness and total sleep time. (Lovibond, Young and Chan , 1988)

Physiologically, elderly adults tend to sleep less and to sleep more restlessly than younger adults, and it is important that the elderly do not have unrealistic expectations about their sleep. (Mullan , Katona and Bellew 1994). Sleep disturbances in old age may have many physical and psychological causes, and if present, any underlying condition should be treated by a physician. As, with insomnia, most recent research concerning stress and/or sleep in the elderly was for testing various hypnotic or other drug therapies. However, several recent researchers pointed out the need for non-pharmacological approaches to sleep therapy. Management strategies can include sleep restriction, diet, exercise, relaxation training ( Nino-Murcia & Keenan, 1988), stimulus control (Engle-Friedman, Bootzin, Hazlewood and Tsao, 1992), sleep hygiene, behavior treatment (Mulla, et. al., 1994), self management (Mendelson, 1987), and meditation/relaxation (Deberry, Davis and Reinhard, 1979). With the exception of sleep restriction, diet and exercise, sound conditioning can play an important role in these therapies to help the elderly achieve better control of their sleep patterns without use of drug.

This review has attempted to summarize and document the important research related to repetitive sound stimulation as an aid to relaxation and sleep in infants and adults, including problem infants (colic), and problem adults (insomnia and the elderly). Certain types of continuous sound have repeatedly been proven to reduce crying in infants; lower heart rate and motor activity in both infants and adults; and induce sleep and improve the quality of sleep.
이 검토는 아이들과 성인들을 평온하게 해주고 수면에 도움이 되도록, 아이들 문제(콜릭), 성인들 문제(불면증과 중장년층) 에 도움이 되는 반복적인 소리와 관련된 중요한 연구들을 요약하고 문서화하기 위함이다.

Based on the foregoing research, it was determined that a sound conditioner using digital recording techniques and micro computer technology with six carefully chosen and recorded sounds would provide an optimum sleep aid for people of all ages. Three sounds were chosen for infants and young children, and three for older children and adults: however, due to individual preferences, any one of these sounds could help relax a person of any age. Additionally , this research pointed out the need to use the instructions packed in each unit to briefly explain some of the secondary findings with infants: specifically, the requirement for relatively loud volume for neonates , and the additional help that swaddling, etc. can provide.

“Womb”
The first sound chosen for infants was a digital recording of an actual ultrasound performed on a woman in the third trimester of her pregnancy. This sound is the rush of the maternal blood flow through the placenta and the umbilical cord to the fetus. Research scores indicated that this was the most effective sound in helping newborns to fall asleep (over 80% asleep in five minutes or less [Murooka, et. al. 1975]). Because it is an actual recording, sounds of the fetal movement and maternal body sounds can occasionally be heard. The name “Womb” was selected to designate this sound.

“Heartbeat”
The sound of the human heartbeat is easily recognized and is believed by most people to be a good sound to help babies sleep. Salk(1962, 1961,1960) proved that it does sooth infants. However, while it is a repetitive sound, it is not continuous and is , by itself, not one of the best sounds for infants, as proven by Brackbill, et. al.(1966). Their research indicated that white noise and lullabies were more effective, and subsequent research indicated that white noise is one of the most effective sounds for soothing infants. Therefore, it was decided that “Heartbeat” could be significantly improved with the addition of white noise as a background for the digitally recorded heartbeat.

“Lullaby”
A lullaby was selected as the third sound for infants, based upon both folk tradition and research such as that by Brackbill, et. al. (1966) most popular lullabies are played on music boxes or high frequency instruments like flutes or violins, or especially today on music synthesizers, and toddlers seem to enjoy listening to these. However, to soothe infants and lull them to sleep, we followed the research of Birns, et. al. (1965), Weiss(1934) and Stubbs (1934) which indicated that, in all cases, lower frequency sounds were significantly more soothing than higher frequency sounds. To offer a lower frequency lullaby, we selected a digital recording of classical guitar, playing an original composition entitled Lexie’s Lullaby (1995 Randy Drew, Conceptual Music Productions). This lullaby should be excellent for infants beginning at two to three months of age when they begin to recognize social sounds (Kopp, 1970)

Ocean surf has been one of the most popular “sound conditioning” sounds since it was first offered by us more than 20 years ago. Very recent research proves that it soothes and relaxes people, even under severe levels of stress, and helps them fall asleep faster and achieve deeper levels of sleep (Williamson, 1972). Scientifically proven for adults, “Surf” can also help youngsters fall asleep.

“Rain”
Rain is the nature sound that is closest to white noise as used in much of the foregoing research, most people will say that they love to fall asleep to the sound of rain, and they do not need research to tell them it works. However, all of the research proving white noise is an excellent sound conditioning sound can be applied to “Rain.” Rain has also been popular in our sound conditioners for over 20 years.

“Brook”
Traditional folklore often has people falling asleep by a mountain stream or bubbling fountain. The digital recording of a babbling brook was selected by a panel of experts at Marpac for its soothing capabilities, and because of focus group feedback. While not one of the sound that researcher have used in their studies, “Brook” certainly fits the definition of continuous, repetitive sound that most researchers say is effective.
