(1)Supraspinatus tendinitis test ?method 환자를 의자에 앉히고 resistance를 이겨낼 수 있도록 abduction을 시킨다. ?의의 supraspinatus tendon의 insertion part의 pain은 그 자체의 degeneration tendinitis을 의미한다. (2)Apley scratch test ?method 환자를 의자에 앉히고 pain side 손으로 occipital region에서 반대쪽 scapular superior angle을 만지게 한다. 다음은 그 손을 back 쪽으로 돌려서 반대쪽 scapular inferior angle 을 만지게 한다. ?의의 통증이 심해지면 건판 중 하나인 supraspinatus tendon이 degeneration tendinitis인 것을 의미한다. (3)Subacomial pushing button syndrome ?method 환자를 의자에 앉히고 subacromial bursa를 pressure한다. ?의의 local pain은 subacromial bursitis일 가능성을 말한다. (4)Shoulder dislocation DUGAS test ?method 환자를 의자에 앉히고 손을 반대쪽 어깨에 대고서 elbow가 chest에 닿게 한다. ?의의 pain 때문에 손을 댈 수 없을 때는 shoulder dislocation이 있는 것이다. (5)Sterno-clavicle joint의 sitting position mobilization ?indication 치료사는 환자가 어깨를 들어 올릴 때 movement의 deficit이나 sterno-clavicle joint에 일어나는 click sound를 확인 한다 이땐 꼭 mis-alignment가 있는 것은 아니다. sterno-clavicle joint에 nodule이 palpation된다. ?concept 환자는 sitting position을 취하고 치료사는 환자의 치료할 부위 뒤쪽에 선다. prime mover hand의 finger로 clavicle위의 medial에서 lateral쪽으로 soft tissue mobilization을 한다. 치료사의 forearm으로 치료할 부위의 humerus를 support 한다. contact을 maintain하기 위해 support hand의 thenar로 prime mover hand를 wrapping한다. 치료사의 chest로 환자의 thoracic cage 뒷면을 fix한다. 환자의 shoulder girdle sterno-clavicle joint가 열릴 때까지 posterior와 superior로 회전시킨다. 이때는 moment adjust를 해도 좋다. ?환자의 치료부위 sternum에 닿아 있는 clavicle의 proximal region ?치료사의 contact point prime mover hand-index finger 2nd, 3rd의 phalanges ?치료방향(line of correction) medial측에서 lateral측으로, 약간 inferior에서 superior쪽으로 (단, 상방에서 sub-dislocation 되었을 때는 medial측에서 lateral측으로 superior에서 inferior쪽으로) ?indication clavicle의 proximal head가 반대편보다 superior에 있다. s-c joint에 nodule이 있다. ?concept 환자는 sitting position으로 있고 치료사는 환자의 sound side 뒤에 선다. contact hand의 thenar part로 superior에서 inferior로 soft tissue mobilization을 한다. prime mover hand의 finger의 처음 contact orientation은 inferior direction이다.
prime mover hand의 finger는 sub-axillar를 향해 회전력(torque power)을 준다. 이때 반대 측 손으로는 환측의 팔을 70도 내지 90도 각도로 abduction 시킨다. 치료방향(line of correction)을 따라 moment adjust하고 회전력(torque power)을 준다. ?환자의 치료부위 clavicle의 proximal head의 upper part ?치료사의 contact point thenar ?치료방향(line of correction) right side는 손상부위는 superior direction에서 inferior direction으로 그리고 clockwise direction으로 회전력(torque power)을 준다. left side은 counter clockwise direction으로 (torque power)을 준다. ?indication shoulder joint dislocation ?concept 환자는 sitting position을 취하고 치료사는 환자의 infect side에 선다. infect side의 wrist와 90도 정도로 굽힌 elbow를 잡는다. support hand로 shoulder를 superior에서 inferior으로 mobilization 한다. 이때 shoulder joint를 external rotation 시킨다.
external rotation이 유지되고 있는 동안 shoulder를 내전시킨다. 이 때 scapular humerus joint의 dislocation이 deduction된다. 환자의 손은 이 때 support를 위해 반대편 shoulder에 놓는다. ?환자의 치료부위 infect side의 구부린 elbow와 wrist ?치료사의 contact point usable both hand (8)Sitting position humerus inferior subluxation adjustment technique ?indication 이 동작은 humerus joint에 inferior direction에서 superior direction으로의 movement deficit가 있는 것을 알 수 있다. humerus이 clavicle distal과 비교해서 inferior direction에 있는 것처럼 보여 질 것이다. pain point는 상완관절와의 앞쪽에 있다. ?concept 환자는 sitting position을 취하고 치료사는 환자의 back에 선다. injury된 part측의 hand로 환자의 forearm을 잡고(팔꿈치 바로 아래) 환자의 팔을 완전히 external rotation 시킨다. injury된 part측의 hand가 팔꿈치를 굽히는 동안 반대편 손으로 환자의 팔꿈치를 잡고 환자의 internal rotation 시킨다.
humerus joint fossa에 tension point를 생기게 하기 위해 inferior direction에서 superior direction으로 상완을 끌어 올리는 동안 chest로 scapular를 fix한다. 빠른 moment adjustment를 inferior direction에서 superior direction으로 한다. ?환자의 치료부위 elbow ?치료사의 접촉점 calcaneal ?치료방향(line of correction) inferior direction에서 superior direction으로 약간 lateral에서 medial direction으로 ?indication 환자는 normal movement에 결함이 있으나 humerus는 항상 정상적으로 보여 진다. ?concept 환자는 prone position을 취하고 치료사는 환자의 치료할 부위에 선다. prime mover hand의 pisiform을 humerus의 posterior에 대고 post direction에서 ant direction으로 pressure를 가한다. 반대편 손으로 환자의 humerus를 약간 internal rotation시키고 tension이 느껴질 때까지 humerus를 extension시킨다.
이때 prime mover hand로 post direction에서 ant direction으로 짧고 빠르게 moment adjustment를 한다. 반대편 손으로는 아무런 moment adjustment도 가하지 않는다. ?환자의 치료부위 humerus head의 post ?치료사의 contact point pisiform ?치료방향(line of correction) post direction에서 ant direction으로 (10)Foozen shoulder mobilization technoque ?indication 환자는 humerus joint fossa에 확실한 movement의 deficit이 있다. ?concept 환자는 의자에 앉거나 선다. 치료사는 환자의 치료할 부위에 무릎을 구부리고 앉거나 선다. (단, 환자의 어깨가 시술자의 어깨보다 높지 않게 하여야 최대한 신전할 수 있다.) 치료할 부위 humerus distal part를 치료사의 shoulder에 올린다.
치료할 부위 shoulder의 scapular superior angle and joint에 prime mover hand의 thenar를 contact시키고 그 위에 보조수의 어제부를 포갠다. 접촉한 환측 어깨에 하방으로 압력을 가하고 치료할 부위 humerus를 받치고 있는 어깨를 들어서 치료할 부위 arm을 abduction시킨다.
이러한 동작을 환자가 참을 수 있을 때 까지 repeat한다. 이 때 resistance mobility end feel에서 moment adjustment해도 된다. ?환자의 치료부위 acromion의 ant,post side concave한 부분 ?치료사의 contact point thenar ?indication scapular가 medial에서 lateral direction으로 moving할 때 motion deficit이 있다. scapular ant에 dull pain이 있다. ?concept 환자는 환측을 above해서 sidelying position으로 하고 치료할 쪽 humerus을 chest를 따라 놓아서 scapular을 돌출시켜 contact하기 쉽게 한다. 치료사는 환자의 전방에 선다. prime mover hand로 medial에서 lateral direction으로 soft tissue mobilization을 하여 humerus joint fossa 와 scapular-clavicle joint 위에 있는 support hand로 fix한다.
medial에서 lateral direction으로 그리고 회전력방향(torque power orientation)으로 짧고 빠른 moment adjustment를 한다. ?환자의 치료부위 scapular inferior angle의 medial aspect ?치료사의 contact hand prime moving hand -pisiform ?치료방향(line of correction) 치료할 쪽이 left side일 경우에 치료할 쪽 medial에서 lateral direction으로 clockwise의 torque power를 준다. 치료할 쪽이 right side일 경우에 counter clockwise으로 torque power를 준다. (12)Sidelying scapular inferior angle external rotation subluxation adjustment technique ?indication scapular가 lateral에서 medial direction으로 움직일 때 motion deficit이 있다. scapular 전방에 dull pain이 있다. ?concept 환자는 치료할 쪽이 above로 오도록 side lying position을 취하고 back posterior로 치료할 쪽의 팔을 돌려 scapular을 돌출시켜 contact하기 쉽게 한다. 치료사는 환자의 전방에 서서 prime moving hand로 lateral에서 medial direction으로 soft tissue mobilization을 한다.
치료사는 accurate adjustment orientation을 얻기 위해 환자의 팔 바로 밑에 prime moving hand를 갖다 댄다. humerus joint fossa와 scapular-clavicle joint 위에 있는 support hand로 fix한다. lateral에서 medial direction으로 그리고 torque power방향으로 짧고 빠른 moment adjustment를 한다. ?환자의 치료부위 scapular inferior angle의 lateral aspect ?치료사의 contact point prime moving hand의 pisiform support hand의 palm은 scapular-clavicle joint에 놓고 손가락은 humerus head에 둔다. ?치료방향(line of correction) 치료할 쪽이 right side일 경우에 lateral측에서 medial direction 그리고 clockwise으로 회전력(torque power)을 준다. 치료할 쪽이 left side일 경우에 counter clockwise로 회전력(torque power)을 준다. (13)Prone position scapular sternum joint adjustment technique?indication scapular가 sternum에 adhesion되어 있고 general motion이 떨어져 있는 경우 ?concept 환자는 prone position을 취하고 치료할 쪽의 arm을 back post로 돌려놓는다. 치료사는 환자의 치료할 쪽에 선다. prime moving hand로 scapular 가장자리를 wrapping 하고 support hand를 prime moving hand hand 앤미에 포개어 support한다.
drop을 이용하여 caudal에서 cephalic으로 cephalic에서 caudal으로 medial에서 lateral direction으로 lateral에서 medial direction 으로 각 3-4회 adjustment한다. 그 다음 clockwise, counter clockwise으로 회전력(torque power)을 가하여 3-4회 drop를 이용하여 adjustment한다. ?치료사의 contact point palm과 finger ?환자의 치료부위 scapular medial border 과lateral border (14)Prone position inter scapular vertebrae muscle의 mobilization ?indication scapular가 thoracic vertebrae에 adhesion되어 있고 general motion이 떨어져 있는 경우 ?concept 환자는 prone position으로 눕고 치료할 쪽의 팔을 등 뒤로 돌려놓는다. 치료사는 환자의 치료할 쪽에 선다. support hand로는 scapular는 돌출시켜 contact하기 쉽게 하기 위해 등 뒤로 돌린 치료할 쪽의 어깨를 anterior에서 posterior direction으로 external rotation 하듯이 살짝 들어준다.
prime moving hand의 digital이나 palm쪽으로 scapular medial aspect를 따라서 medial측에서 lateral direction으로 밀어 준다. ?환자의 치료부위 scapular medial aspect muscle부위 ?치료사의 contact point palm 혹은 digital 주관절(Elbow joint) 교정법 (1)lateral epicondylitis test (cozen test)?method 환자를 의자에 앉히고 고정시킨 채 주먹을 쥐고 손목을 뒤로 젖힌다. 다음 손목을 굴곡시키도록 압박을 하면서 반대로 저항하도록 한다(M.E.T) ?concept lateral epicondyle과 fist에 생기는 pain은 lateral epicondylitis가 있음을 의미한다. (2)Medial epicondylitis test ?방법 환자를 의자에 앉히고 팔꿈치를 굽히고 팔을 회외시킨 상태로 환자는 팔꿈치를 펴게 하고 치료사는 펴지 못하게 서로 저항하게 만든다.?의의 medial epicondyle에 pain이 있으면 medial epicondylitis이 있는 것으로 본다. (3)Supine position elbow joint traction technique ?indication humeroradial 또는 humeroulnar 의 injury especially, elbow joint의 extension limit of motion이 있을 때 ?concept 환자는 supine position으로 눕고 치료사는 환자의 치료할 부위에 선다. wrist를 elbow fossa에 superior에서 inferior로 soft tissue mobilization을 하여 contact한다. 다른 한 손은 elbow joint의 extension이 느껴질 때 까지 patient의 wrist를 shoulder 쪽으로 approach시킨다. 이러한 과정을 환자가 참을 수 있을 만큼 repeat한다. ?환자의 치료부위 환자의 elbow fossa ?치료사의 contact point 치료할 쪽에 놓는 치료사의 wrist (4)Radius의 posterior subluxation adjustment technique ?indication humeroradial joint에 관절액의 움직임이 소실되었을 때 radius head에 nodule point가 있다. ?concept 환자는 sitting position을 취하고 치료사는 환자의 치료할 부위옆에 앉거나 standing position을 취한다. 한쪽 thumb digit으로 radius head의 posterior에서 anterior direction으로 soft tissue mobilization을 하고 다른 한손으로 환자손목의 손등면을 잡는다.
elbow joint를 가볍게 complete extension이 이루어지기 전 까지 extension 시킨다. 이때 손목을 internal rotation 시키고 elbow joint를 extension 시키면서 빠르고 짧게 posterior에서 anterior direction으로 moment adjustment한다. ?환자의 치료부위 radius head의 posterior aspect ?치료사의 contact point thumb digit ?치료방향(line of correction) posterior에서 anterior direction으로 |
