집에 고양이를 5마리를 키우다보니...

피부염 곰팡이 감염으로 돈이 들어가서...

정리해봄 ( 피부과 의사는 아니다냥)

1. Ringworm (  tinea,백선증 )  원인 ?   

  fungus(곰팡이) 감염

2. Ringworm (  tinea,백선증 )  감염 형태 ?   

● Ringworm (  tinea,백선증 ) 인수공통감염

인가 ?       

 곰팡이 감염이니까  cat, dog, human 모두 서로 감염됩니다. 

3. Ringworm (  tinea,백선증 )감염 부위

  ● Hair

•Tinea capitis – Infection of scalp hair

                             (두피의 머리카락)

•Tinea barbae – Infection of beard hair

●Epidermis

 • Tinea faciei – Infection of the face

• Tinea corporis(체부백선) – Infection of body surfaces other than the feet, groin, face, scalp hair, or beard hair

•Tinea pedis – Infection of the foot

( athelete's foot,  tinea of foot  발백선, 무좀 )

•Tinea cruris (샅백선)– Infection of the groin, proximal inner thighs, or buttocks

•Tinea manuum – Infection of the hand

●Nails

•Dermatophyte onychomycosis (tinea unguium, 손톱 백선)

● Tinea versicolor ( pityriasis versicolor,  전풍, 어루러기) 

-tinea versicolor 의 pathogen ? saprophytic yeasts in the genus Malassezia (formerly known as Pityrosporum).

Malassezia is a lipid-dependent, dimorphic fungus that is a component of normal skin flora.

Transformation of Malassezia from yeast cells to a pathogenic, mycelial form is associated with

the development of clinical disease.

● Tinea nigra (흑색백선증)

- tinea nigra 의 pathogen ?    the dematiaceous, yeast-like fungus Hortaea werneckii.

* tinea corporis 

* Tinea versicolor (전풍 어루러기 ) : hyperpigmentation or hypopigmentation

* tinea nigra 

이놈은 dermoscopy로 봐야한다함.. melanoma랑 감별..

3. Ringworm 진단 접근과 치료 

1) Ringworm 검사 ?   KOH 검사

(skin에서 긁은 조직을  KOH용액 처리후 현미경 검사에서  fungus Hyphae 확인하면 확진 )

긁은 조직을 알카리성 KOH 용액에 담그면

 모두 녹고 곰팡이만 남은다고 배웠는데 피부 squamous cell이 아래 그림에서 보이네요 ㅋㅋ

2) Ringworm 치료 

(1)  ( Widespread면 ) fungus medication or

( localized되있다면 ) topical antifungal therapy ( 항진균제 연고 등)

   => itraconazole, terbinafine, fluconazole 

      성분 들어있는 연고나 약물 복용

(2)  Ringworm으로 인한 2차 세균감염 예방 약 

 즉 항생제

(3)  itching을 완화시키는 low potency steroid

 사용

* medium- or high-potency corticosteroids 은 사용하지 맙시다.

* Ringworm에 효과없는 항진균제 (antifungal drug)  ?    Nystatin 

3) cat, dog ringworm 치료시 주의점 ? 

  Nystatin 들어있는 약 사용 하지 말것

( 인터넷에서 고양이 ringworm 치료로 

비싼 Nystatin + neomycin + steroid

병합제제를 판매하는데 사지 마시구요..)

* cat, dog  ringworm치료 : antifungal

                  + antibiotics

                   + itching완화하는

                       low potency steroid

(1)- 사람이 사용하는 Itraconazole, terbinafine 들어있는 무좀 연고  ? 

 ㄱ. Terbinafin( 라미렉스, 나니실크림, 라미실,

                         라미실덤겔, 라니텐, 로시놀 등등 ),

ㄴ. 먹는약은 feline 찾아보니 고양이 경우

Fluconazole 1회용량 10mg/kg을 2회

복용시키고 호전시까지 복용인데 수의사와 부작용 상의하고 복용 시키세요.

전 의사지 수의사가 아닙니다.

바르는것은 문제없는데 먹는것은 수의사에게..

사람은 간독성 생겨서 술, 생약,한약 등 주의해야함.

(2)- ring worrm 으로 인한 2차 세균감염예방

  항생제  연고 ? 

        ㄱ. 테라마이신 연고 ( Oxytetracycline HCl 5mg, Polymyxin B Sulfate 10000I.U)

        ㄴ.   네오덱스연고 (   Dexamethasone 1mg,  Neomycin Sulfate 3.5mg, Polymyxin B Sulfate 6000IU   )

         ㄷ.  데카미솔 연고 ( Centella titrated ext. 10mg   1g 중   Hydrocortisone acetate 10mg, Neomycin Sulfate 3.5mg )

         ㄹ.   후시딕 항생제 연고 + 스테로이드 연고 

등등 항생제와 스테로이드병합연고

예를들어서 

그럼 무좀연고랑 항생제 연고를 같은 양으로 짜서 고무장갑을 끼고 섞어서 고양이 몸에 발라주세요..

당연 고양이가 못핧게 해주셔냥.
