Impulse or physiological venous pump action could reopen critically closed capillaries and reduce tissue pressure either by causing reflux of blood into the venous limbs of capillary loops, enabling tissue fluid to be reabsorbed by osmosis, or by increasing blood perfusion pressure thus causing relaxation of the smooth muscle of the arteriolar resistance vessels.
impulse나 생리학적 정맥 pumping은 심각하게 막혀있던 모세혈관을 열 수 있고 조직압을 낮출 수 있다. 이는 모세혈관의 정맥부분에 혈류의 재유입을 일으켜서 조직액이 삼투압으로 인해 재흡수되거나 혈류 분포압을 높여 저항 동맥이 있는 평활근의 이완으로 이룰 수 있다.
It is now established that rapid pressure changes such as those produced by weight-bearing on the foot, or by impulse pumping, are followed by increased blood flow into the limb due to widespread diminished vascular resistance. Such hyperaemia lasts about 20sec; increases in popliteal artery flow of 93% in normal subjects and 84% in patients with peripheral vascular disease have been recorded (Gardner and Fox 1989). These increases are totally out of proportion to the slight diminution of flow recorded during the actual compression.
발에 가해지는 무게 부하나 impulse pumping에 의해 발생한 급한 압력 변화는 광범위하게 감소하는 혈관 저항성에 의해서 사지의 혈류가 증가한다는 이론이 확립되어있다. 그러한 울혈은 20초 정도 지속된다; popliteal 동맥(오목동맥)의 혈류가 일반 대상자에서는 93%가 증가했고 말초혈관질환을 앓고 있는 환자에서는 84%가 증가했다고 기록되었다.(Gardner and Fox 1989). 이러한 증가는 실제 압력을 가하는 동안에 발생한 약간의 혈류 기록 감소과 비해서 절대 비례하지 않았다.
Hyperaemia after impulse compression is probably mediated through endothelial-derived relaxing factor (EDRF). EDRF is nitric oxide with a half-life of a few seconds; its effect is localised to the tissues and is not systemic. It is liberated by sudden pressure changes (Yield Shear Stress) on endothelium and it diffuses locally and rapidly to relax vascular smooth muscle(Editorials, Lancet 1987, 1988).
impulse 압박 후에 발생한 울혈은 내피세포기원 이완요소(EDRF)를 통해 매개될 것이다. EDRF는 몇 초의 반감기를 가진 산화 질소(NO)이다; 그 영향은 전신이 아닌 조직으로만 국한되어있다. EDRF는 혈관 내피에 대한 급작스러운 압력 변화에 의해 퍼지게 되고 국소적으로 빠르게 분포하며 혈관의 평활근을 이완시키게 한다.
Since the hyperaemic effect is dependent on venous priming, EDRF is probably produced by pressure changes in the smaller venous radicles and then diffuses locally to the closely accompanying arteriolar resistance vessels (Tangelder, Slaaf and Reneman 1984) which it temporarily dilates to increase blood flow. In compartment syndromes an increase in perfusion pressure would encourage the reopening of critically closed capillaries enabling reabsorption of tissue fluid to resume and compartment pressure to fall.
울혈 효과는 정맥의 혈류에 의존하고 있기 때문에 EDRF는 작은 정맥 혈관의 압력변화에 의해 발생하며 그 후에 주변에 있는 저항동맥에 국소적으로 퍼지게 될 것이며 (Tangelder, Slass and REneman 1984) 임시적으로 확장되어 혈류를 증가시킬 것이다. 구획 증후군에서 분포압의 증가는 심각하게 폐쇄된 모세혈관을 여는 데 도움이 될 것이고 그로 인해 조직액의 재흡수가 재개되며 구획의 압력이 떨어지는 것을 가능하게 한다.
