Artifact를 줄이는 요령
접지는 자극전극과 기록전극 사이에 붙인다.
전선들은 침대위에서 겹쳐지지 않게 하고, 가급적 서로 멀리 떨어지게 둔다.
자극 세기 (mA)는 가급적 약하게 해야 파형이 잘 나온다. 특히 감각신경.
자극전극의 음극의 위치에 따라서도 파형이 다르게 나타난다.
핸드폰을 먼곳에 두기.

목적과 기능

판독
1. Localize the lesion site (Neruon, axon, NMJ, or muscle)
2. The fiber type involved (sensory or motor)
3. Underlying pathology (axonal or demyelinating)
4. Severity and temporal course of the disease

---------------병변의 위치, 운동신경/감각신경 감별, 수초/축삭 감별, 경과 시간, 심한 정도

Severe weakness 증상이라면 root lesion은 아님
Small fiber 는 NCS로 검사할 수 없다. Pain temp 의 증상만 있고 검사가 정상일 때에는
small fiber neuropathy이다.

Motor CMAP  200mV, 15ms
Sensory SNAL 50 uV
Motor nerve 는 2 군데의 latency를 측정해야 NCV 계산가능. Sensory nerve는 1군데로도 충분.

Sensory neve는 정상적으로 temporal dispersion과 phase cancellation이 생긴다.
Demyelination 질환은 entrapment 외에는 드물다.

Axon loss 시 NCV와 latency는 정상이어야 하지만, NCV 는 75% 범위 이내로 감소, latency는 130% 이내로 연장.

위의 사항 외 판독 순서
1. 먼저 CMAP amp 를 보고 axon인지
두 군데 CAMP 를 비교하여 conduction block이 있는지 (demyelination 인지)
2. Wallerian degeneration 시기:
Motor 는 5 days, Sensory는 10 days

1. Diffuse CMAP amplitude (if decreased, axonal injuiry) (Wallerian degeneration in motor:5 days, sensory:10 days)
2. Conduction block (Significant difference of CMAP amplitude between distal and proximal) (if exist, demyelination)

Radiculopathy
Median and ulnar F-wave는 C8-T1 nerve root 를 지난다.

Fibrilltion & PSW 나타나는 순서
10-2주; paraspinal ms
2-3주: Proximal muscles
3-4주: lower leg muscles
5-6주: foot muscles
수 개월후 reinnervation에 의해 paraspinal ms의 PSW가 없어짐.

EMG로 radiculopathy 진단시 어려운 점
1. A single root level 의 radiculopathy는 진단곤란.
2. 2주 전에는 정상
3. Demyelination 뿐이라면 진단 곤란
4. Sensory n 만 병변이라면 정상.
5. different Fascicle may be preferentialy affected or spared.
6. 수 개월후 reinnervation 발생후라면 plexopathy와 구별 곤란.
7. Segmental level까지 진단하기 위해서는 limb ms 까지 근전도 해야함.
8. Radiculopathy, polyradiculopathy, focal or diffuse motor neuron ds 의 소견이 같다.
9. Paraspinal muscle 의 fibrillation potentials 는 radiculopathy 뿐 아니라 proximal myopathy, motor neuron disease, neuropathy affecting the dorsal rami (diabetic polyneuropathy)에서도 보인다.
10. 노인에게서 mild chronic distal polyneuropathy 와 mild chronic bilateral L5-S1 radiculopathy를 구별하기는 불가능하다.

손이나 발 또는 얼굴의 감각이 둔하거나, 힘이 빠진다면,
말초신경에 문제가 있을 가능성이 큽니다.
말초신경은 전깃줄 처럼 가늘게 뻗어 있으므로,
CT 나 MRI 촬영으로는 말초신경을 볼 수 없습니다.

그런데, 말초신경은 전기줄과 비슷하게
전기를 잘 통하는 성질이 있기 때문에, 
이런 원리를 이용한 것이
신경 전도 검사입니다. 

신경 전도 검사는,
말초 신경의 한 쪽에 전기 자극을 주고,
다른 쪽에 전기가 도달한
속도와 세기를 측정하여,
말초 신경에 문제가 있는지를
확인하는 검사입니다.

그런데, 팔, 다리, 얼굴에서
전기 자극을 줄 수 있는 부위는
몇 군데 되지 않기 때문에
더욱 자세한 검사를 하기 위해서는
근전도 검사가 필요한 경우도 있습니다.

신경 전도 검사를 할 때에는
전기로 자극을 하는 순간
따끔하게 아플 수 있습니다.
그렇지만 전기의 세기는 30 mV 이하여서,
어린이도 견딜 수 있는 정도이므로
안심하셔도 됩니다.
단, 심장 박동기를 하고 계신분은
신경 전도 검사를 하실 수 없습니다. 

만약 검사 예약을 하신다면,

얼굴에 검사를 받으실 때에는
가급적 화장을 말고 오십시오,
발에 검사를 받으실 때에,
땀이 너무 난다면
가급적 깨끗히 씻은 후
검사를 받으십시오.

근전도 검사를 할 때는
근전도 바늘이 피부를 통과할 때,
따끔하게 아플 수 있습니다.
근전도 바늘은 아주 가늘어서,
조직 손상이 아주 경미하여, 
피가 나는 경우도 거의 없으므로,
염려하지 않으셔도 됩니다.

아스피린을 복용하시는 분은
검사를 받으실 수 있으나, 
혈액응고와 관련된 질환이 있거나
항응고제를 복용하시는 분은
출혈의 위험이 있으므로
가능하면 근전도 검사를
하지 않고 있습니다.

근전도 검사는
근육에 힘을 완전히 빼고 있을 때
정확한 결과가 나옵니다.

허리의 근육을 검사할 때에는
옆으로 누워서
마치 뱃속의 어린아이 처럼
허벅지를 배에 붙이면서
허리를 최대한 굽히고
숨을 천천히 내쉬면,
허리에 힘이 빠집니다.  
목의 근육을 검사할 때에도,
옆으로 누워서
목을 최대한 굽히십시오.

간혹, 근육에 아주 약하게 힘을 주면서
검사를 할 때도 있습니다.
이 때에는
근전도 기계에서 나오는 소리를 들으시면서

검사자의 지시대로 하십시오.

If any of the ulnar muscles are abnormal, test the following muscles:
At least two non-ulnar lower trunk/ T8-T1 : abd poll brev, Fl poll long, Ext indicis proprius
C8-T1 paraspinalis

At least two post interosseous-innervated muscles :  Ext indicis propri, ECU, Ext Digit Profundus
At least two radial-innervated muscles proximal to the post interosseous nerve, distal to the spiral groove : Brachioradialis, Ext carpi radialis.
At least one radial-innervated muscles proximal to the post interosseous nerve, distal to the spiral groove : Triceps, anco.
At least one non-radial post cord-innervated muscles : Deltoid, Latissimus dorsi.
At least two non-radial C7-innervated muscles: Fl carpi rad, pron teres, Fl poll long, Fl digit sublim,
C-paraspinalis.
