Cancer[암]
Dietary Supplements for Support During Cancer
Introduction[개요]
In 1999, more than 1.2 million new cases of cancer were detected and cancer killed almost 600,000 Americans ? almost evenly split between men and women. For women, breast cancer is the obvious main concern (175,000 women will deal with this disease each year), while for men, prostate cancer is the second most preval!ent form (180,000 cases and 37,000 deaths in 1999) following lung cancer. Because a recent study in Cancer (2000:88:615-19) suggested that as many as one-third of cancer patients supplement their conventional medical treatment with alternative therapies, such as dietary supplements, it is import!ant for both physicians and patients to understand how dietary supplements may help or hinder conventional oncology treatments.
What is cancer?[
Cancer, aside from being one of the most dreaded words in any language, can be defined as the uncontrolled growth of abnormal cells derived from normal tissues. The aspect of cancer that can make it so deadly is that it may eventually cause death by spreading from the site of origin to other sites within the body and causing organ failure. There are approximately, 200 different types of cancer ?and the following information will do little more than scratch the tip of this deadly iceberg. For example, cancers can arise from the epithelial cells that line body cavities (carcinomas) or from the mesenchymal cells that make up connective tissues, blood and the lymphatic system (sarcomas). Perhaps the most simplified way to describe cancer is as a cellular malignancy that leads to unregulated growth, lack of differentiation and ability to invade and spread to surrounding tissues (metastasize).
What causes cancer?[
Perhaps the very next question after learning about what cancer IS, comes the inevitable question about what causes cell growth to go awry in the first place. The short answer is that there are lots of things that promote cancer development and, besides smoking and poor diet, it is difficult to point out any single lifestyle factor as a dominant contributor. Instead, it is often instructive to understand a bit about the epidemiology of cancer to help outline the who/what/why about cancer risk. Here are a few statistics to consider.
Age has the most significant impact on the incidence of and mortality from cancer. For those who live in the United States, the incidence of cancer doubles every five years after the age of 25 ? with almost 70% of all cancer deaths occurring after the age of 65. In particular, cancers of the prostate, stomach and colon appear to be the most age-specific ?with the highest rates between the ages of 60 and 80. In contrast, children age 10 or under appear to have a great incidence of blood cell cancers such as leukemia (acute lymphoblastic leukemia).
Cancer risk follows a geographic pattern ? so that certain parts of the country and the world exhibit greater rates of specific cancers. For example, the risk of breast cancer in Japan is quite low, while the average American woman has a 1-in-8 chance of getting breast cancer in her lifetime. Almost exclusively due to cigarettes smoking, worldwide rates of lung cancer have increased steadily over the past 7 decades so that now nearly half of all premature deaths in the western world are tobacco related. Scary, isnt it? Across the United States, the mortality from cancer appears to be higher in the New England and Mid-Atlantic States, while being lower in the West and Southwest. Overall, the American Cancer Society pegs the annual cancer death rate in the United States at 174 per 100,000.
Gender plays an obvious role in the development of certain cancers ? after all, men do not have breasts and women do not have prostate glands. For men, prostate cancer is the major concern ? occurring in about 1 out of every 8 men over a lifetime. For women, breast cancer also occurs at a rate of about 1 in 8 over a lifetime. Both of these cancers are particularly insidious in that they can strike a man or woman in the prime of their lives. Lung cancer, however, is an equal opportunity killer and is the most common cause of cancer death in both sexes (although colon cancer is runner-up to lung cancer in terms of both incidence and mortality across the genders).
It is also quite true that family history can play a role in the development of certain cancers. The problem is, that even with the ongoing development of powerful gene -screening tools, there is little r(nothing) that we can do about our individual risk of developing an inherited cancer (or other inherited disease). Short of choosing your parents wisely, we all need to rely on the same general lifestyle choices (diet, exercise, supplements) that are presented below.
Causes of Cancer Deaths (United States)[
Type of Cancer : Cause, % of Total [
*Lung(폐), Bladder(방광), Kidney(신장), Pancreas(췌장), Cervix(후두) :
Tobacco(담배), 30-35%.
*Colon(결장), Breast(유방), Pancreas(췌장), Prostate(전립선), Ovary(난소), Endometrium
(자궁), Liver(간) : Diet-related(영양관련), 30-35%
*Skin(피부) : Sunlight tanning beds(일광욕 침대), 2%
Controllable risk factors[
It makes sense that since there is an association of 30 to 35% of cancers are diet related and that tobacco related cancers also cause 30 to 35% of all cancers, that these two controllable risk factors are the place to start. First and foremost on your cancer prevention to do list is simple ? STOP SMOKING (and stop exposing yourself and others to second hand smoke). Next up is to get yourself up and out the door for some exercise. A sedentary lifestyle is associated with a higher risk of many cancers and the simple act of walking a few miles several times each week can help reduce the risk of cancer of the colon, breast, prostate and lung.
The nutritionally linked cancers include esophagus, stomach, liver, colon, rectum, breast, prostate, endometrium and ovary. The dietary aspects most commonly associated with these cancers are pickled foods, salted foods, alcohol, nitrates, mycotoxins, nitrosamines, fried foods and hydroxy radicals (caused by barbecuing). This does not mean that you have to memorize each of these classes of chemicals and then religiously avoid them like the plague. This laundry list is presented simply as a means to help you realize that the simple incorporation of a higher proportion of fruits and vegetables (and the associated phytonutrients and fiber) can go a long way toward reducing cancer risk and helping your body fight any cancers that you may already be battling.
In many cases, people learning of their heightened risk for cancer, or the diagnosis of themselves or a loved one with the disease spurs them to radically change their approach to eating. Some of the more popular diets, including Mediterranean, Asian, Vegetarianism, Macrobiotic, and High Protein (and/or Low Carbohydrate), are outlined below. Several types of low-fat, primarily vegetarian-based diets have very strong evidence for cancer protection due to a reduced exposure to carcinogens (fewer processed foods) and an increased exposure to anti-carcinogenic phytonutrients.
Mediterranean and Asian diets both tend to rely on a semi-vegetarian composition of foods ? lots of fruits and vegetables, small amounts of protein (lots of fish), and little (if any) eggs or dairy products. The key difference between the two diets is that the Mediterranean diet also includes a relatively high intake of olive and flax oils (which supply omega-3 fatty acids), while the Asian diet will have a higher consumption of soy-based foods (which supplies isoflavones). Even though the Mediterranean diet is not a particularly low-fat diet (at about 40% of total calories), breast cancer rates in Greece and Italy are much lower than might be expected. Asian diets, on the other hand, are quite low in total fat content and have also been associated with reduced rates of breast cancer ? possibly due to the anti-cancer effects of isoflavones and other compounds found in tofu, fish, garlic, and green tea.
Vegetarianism is the practice of adopting a diet that includes foods derived exclusively (or almost exclusively) from plants. Vegetarians do not eat meat, although many of the less strict vegetarians may include small amounts of dairy products or eggs into their diets (lacto-ovo- vegetarians). Strict vegetarians, called vegans, eat no meat, fish, poultry, eggs, or dairy products. Because their diets are devoid of the richest sources of protein and vitamin B12, vegans need to be especially careful about combining plant sources of protein (such as rice and beans) in adequate amounts to meet daily protein requirements.
In most cases, a vegetarian diet is healthier than the way most Americans normally eat (largely because a vegan diet incorporates a large amount of fruits and vegetables). Leading health organizations have encouraged the reduction of animal fats and the increase in fruits and vegetables in the American diet for decades. It is well known that excessive saturated fat intake contributes to an increased risk for many cancers as well as heart disease. Most health professionals recognize that diets which rely on whole grains and fresh fruits and vegetables can result in better overall health. It is well-known that vegetarian diets can help reduce obesity and constipation and there is growing evidence that such diets may be effective in reducing the risk of cancers of the lung, colon, breast, and prostate as well.
Macrobiotic diets became popular in the early 1960s as more of a spiritual movement than as a healthy lifestyle. Today, macrobiotic diets still incorporate a high degree of spirituality by combining diet, consciousness and social philosophy, to promote healing and healthful living. Macrobiotic diets are generally based around one of several Eastern philosophies (Chinese and Indian are common) and may incorporate the concept of balancing various universal factors (such as the Chinese/Japanese principle of yin-yang). The yin-yang concept assigns opposite values to every component of life and nature (such as hot/cold, male/female, light/dark) ? with the overall concept that in order to achieve health and happiness, once must first achieve balance in their yin and yang. The macrobiotic diet is intended to be yin-yang balanced, and therefore healthy.
The macrobiotic concept includes more than just a diet or simple food selection. It requires special methods of food preparation as well as the assessment of yin-yang values in each geographic location. In other words, it is a major effort or a paradigm shift away from the typical American diet and lifestyle. Macrobiotic philosophy dictates that only certain pots, pans, and utensils made of certain materials may be used. However, all wood, glass, ceramic, stainless steel, and enameled pieces are allowed. Copper and aluminum may not be used. From a practical standpoint, this diet and philosophical approach can take a lot of planning and may cost more than other dietary approaches.
The typical macrobiotic diet (if there is such a thing) is generally comprised of about 50%-60% whole grains because they are thought to be neutral in the yin-yang continuum. Vegetables make up another 25%-30% of the diet, with the remainder of the diet being various beans, seaweed and soups (miso soup is quite popular). Dairy, eggs, sugar, meats and processed foods are generally not part of most macrobiotic diets. Like the vegetarian diets, a macrobiotic diet may help in the reduction of cholesterol, body weight and heart disease as well as certain cancers ? most notably colon, prostate, and breast cancers.
High Protein/Low Carbohydrate (HPLC) diets have been popular decades ? they come and go as fad diets with the changing of the seasons. You may recognize this type of diet by some of its more popular names such as The Atkins Diet,Protein Power, Carbohydrate Addicts, and many others. The primary reason that most people follow one of these diets is for the rapid and effortless weight loss that is promised. Although there are no direct cancer-reducing benefits of such a diet (aside from an indirect benefit of any weight loss), there may be a very real cancer-promoting side effect from the HPLC diets due to their promotion of high fat intakes.
In most cases, the HPLC diets are extremely low (or devoid) of carbohydrates (less than 20% of total calories) such as breads, rice, pasta, cereals, almost all fruits and certain types of vegetables. This leaves protein and fat from which dieters are encouraged to consume the bulk of their daily calorie needs. Although most of the popular diet books recommend the majority of fat intake to come from unsaturated varieties (poly- and mono-unsaturated oils), this message is lost on the vast majority of dieters who feel that the diets are a license to eat all the saturated fat that they want (think bacon and mayonnaise sandwiches). The chief problem with a high dietary intake of saturated fat (and low intake of fiber) is an increased risk of heart disease and cancer (because certain fatty acids can turn on oncogenes and increase risk for breast cancer, colon cancer and prostate cancer).
Although a HPLC diet may be quite effective for achieving short-term weight loss in some people, much of the loss in body weight will come from body water and glycogen losses and the restrictions on carbohydrates, fruits and vegetables are very difficult to adhere to for any length of time.
What to look for in a diet [
Never follow a diet that eliminates whole food groups. Eliminating whole food groups can increase the chances of a vitamin or mineral deficiency.
Always eat a diet that is varied and not monotonous. Eating a wide variety of foods virtually insures a good consistent intake of many vitamins, minerals and other health promoting agents found in foods.
Avoid eating fast foods. Avoiding fast foods and fried foods is an easy way to keep your fat and caloric intake under control.
Try to eat food that has a low sugar content. This means avoid foods that have a high simple sugar level.
Take a multivitamin / mineral supplement daily. In addition, always inform your health care practitioner of what kind of diet that you follow and any and all dietary supplements that you may take.
Dietary Supplements[
Lets start off with a VERY import!ant point - Dietary supplements cannot prevent or cure cancer on their own. Although some of the supplements outlined below may be beneficial when added to a healthy diet and regular excise program, NONE of them present any sort of miracle breakthroughs in terms of cancer prevention or treatment (despite what the breathless marketing materials might lead you to believe). A large number of clinical trials are underway with select vitamins, minerals, phytonutrients and other supplement ingredients to determine their relative cancer protective properties. This research appears to be promising for selenium, green tea and some other vitamins, but the research to date had largely been conducted in test tubes and lab animals ? and it is FAR from being conclusive regarding the effects in humans.
The bottom-line, in terms of nutrition, for preventing cancer is to eat a diet that is high in fruits, vegetables and whole grains and low in processed meats (smoked/pickled/charred from grilling/salt-cured). That said, the following supplement recommendations are cautiously presented with the recommendation that any cancer patient should work with his/her oncologist to optimize their specific treatment regimen ? whether it includes dietary supplements or not.
PC-SPES is a combination of eight different herbs that appear to have estrogenic activity and have been found to have a positive effect during prostate cancer treatment. Data points to an improvement in quality of life measurements, reduction in metastatic pain as well as a reduction in prostate specific antigen (PSA). PC-SPES has also been found to have a tumor shrinking effect in the animal model. PC-SPES is sold over the counter, through the Internet and at many healthfood stores, however great caution should be used when using or incorporating PC-SPES into your treatment plan. No one should use PC-SPES without the oncologist being aware or approving its use.
Antioxidant nutrients, whether vitamins (C, E + D), minerals (Selenium + Zinc) or phytonutrients (catechins + flavonoids), have shown terrific promise in the prevention of skin cancer (which affects as many as 1 in 4 people). Although most cases of skin cancer are highly treatable (if caught early enough) prevention of the disease in the first place is the better choice. The most serious form of skin cancer, melanoma, used to be quite rare in the United States but rates are increasing faster than any other form of cancer and is expected to affect approximately 1 in every 100 people and account for 9000-10,000 deaths. Because the greatest risk factor for developing skin cancer is sun exposure (and a fair complexion), the best preventive measure is to avoid excessive exposure by using sunscreens, sunglasses, etc.
Several studies have suggested a beneficial role of antioxidants (used both internally as supplements and externally as topical cosmetics) with combinations of vitamins A, C, E, selenium and beta-carotene actually resulting in an increase in the skins own ability to resist sun damage (sort of like an internal sunscreen). The Skin Cancer Foundation recommends that skin cancer risk can be minimized by minimizing your sun exposure between 10AM-3PM and using a sunscreen with SPF-15 or higher (apply to all exposed areas and re-apply every 2 hours).
Vitamin C, the primary water-soluble antioxidant nutrient in the diet, is known to be depleted in people afflicted with various cancers. While vitamin C supplements have shown conflicting results with regard to cancer prevention, its role in regenerating vitamin E (reducing tocopherol radicals) provides support for including vitamin C in any antioxidant supplement regimen. It may be that vitamin C, like beta-carotene (see below), is but one dietary substance needed for overall chemo-protection and that mixed sources (from foods) are your best approach against cancer. Several studies have found a strong association between high dietary vitamin C intake (from foods) and a reduced incidence of cancers of the stomach, bladder, throat, colon and breast ? perhaps via a suppressed formation of carcinogenic nitrosamines in the gastrointestinal tract.
Niacin(Vitamin B-3) is known to increase levels of NAD in various tissues ? an effect which may lead to an enhanced capacity to repair cellular damage that could lead to cancer.
나이아신(비타민B-3)는 여러 세포조직에 있는 보조효소의 일종인 NAD(nicotinamide -adenine dinucleotide)의
Milk Thistle (Silymarin) is a popular herbal supplement for liver detoxification ? but it also possesses powerful effects as an antioxidant. A number of animal studies (rodents) have shown a reduced occurrence of several cancers (perhaps due to an effect on modulating liver enzyme pathways).
Grape seed extract and green tea extract are both rich sources of polyphenol compounds such as proanthocyanidins (in grape seed) and catechins (in green tea) which are potent antioxidants and deliver powerful anti-tumor properties (in rodent models of cancer). Grapes also contain several phytonutrient compounds, such as resveratrol and quercetin, that have potent antioxidant properties.
Sulforaphane and indoles are compounds found in broccoli, cabbage, cauliflower and related cruciferous vegetables, that are thought to possess anticancer properties by enhancing the livers ability to detoxify certain carcinogens more effectively.
Isoflavones, such as daidzein and genistein, (from either soy or red clover) possess both antioxidant and weak estrogenic activity. The anti-estrogen effect of small doses of isoflavones may be beneficial in certain forms of hormone-sensitive cancer, such as breast, uterine and prostate, but very high doses could theoretically increase cancer cell proliferation and promote tumor growth. For this reason, it may be prudent to consume isoflavones supplements at levels that are achievable in a normal diet high is isoflavones-containing foods (probably no more than 50-100mg per day). We know from animal studies that soy-based diets are associated with reduced cancer risk, so it may be speculated that a similar effect may be observed in humans.
Vitamin E, as the major fat-soluble antioxidant in many parts of the body, can be quite effective in protecting delicate structures in the skin and eyes from the damaging effects of sun-induced oxidative stress. Several animal studies have shown vitamin Es ability to reduce skin cancer (both orally and topically) and human population studies suggest a beneficial role of vitamin E in preventing prostate cancer (400-800IU/day). In one human study, 50mg of vitamin E (synthetic form) reduced the incidence of prostate cancer by more than 30% and the death rate from prostate cancer by more than 40%, while colon cancer was reduced by 16%. In other studies, colon cancer risk has been cut by 30-70% depending on the dose and duration of vitamin E supplementation (usually around 200IU per day) and positive benefits have been noted for vitamin supplements (400-800IU/day) in cancers of the liver, mouth and stomach (but perhaps not against lung cancer).
Selenium deficiency has long been known to be associated with an increased risk of several forms of cancer and supplements (200mcg/day) are thought to protect against cancers of the prostate, colon and lung (but probably not against skin cancer). In one of the best studies, subjects received either 200mcg/day of selenium or a placebo. Those receiving the selenium supplements showed a dramatic reductions in cancer of the prostate (66%), colon (50%) and lung (40%) as well as an overall 50% reduction in cancer deaths. The best supplemental sources of selenium should supply 100-200mcg/day in a bioavailable form such as selenomethionine, selenium-rich yeast, or high-selenium garlic (instead of an inorganic form such as sodium selenite ? which may be found in many basic multivitamins). Caution is urged at higher doses of selenium, which can lead to toxic effects at doses above 900mcg-1mg per day (gastrointestinal distress, loss of hair/fingernails, forgetfulness, etc.).
Garlic may be helpful in reducing the risk of colon cancer by as much as 30% - possibly via either its antioxidant properties (garlic is high in selenium) or through its ability to influence certain detoxification enzyme pathways in the liver. Several population studies conducted in China, Europe and the Unites States have shown that people with the highest dietary consumption of garlic have a significantly reduced risk of developing colon cancer.
Beta-Carotene, although possessing powerful antioxidant properties, may NOT be the ideal supplement for cancer prevention. In fact, there is some fairly strong evidence that although mixed carotenoids from fruits and vegetables may provide a cancer preventive effect, high-dose supplements of purified beta-carotene may actually promote certain forms of cancer in specific populations (lung cancer in life-long heavy smokers). We know from population studies that lung cancer risk is reduced by almost 75% by consuming a diet rich in fruits and vegetables (particularly cruciferous veggies like broccoli and cauliflower), but at least 2 very large intervention studies conducted in the mid-90s, the Alpha-Tocopherol, Beta-Carotene (ATBC) study and the Beta-Carotene and Retinol Efficacy Trial (CARET), showed a clear 18-46% increase in lung cancer among smokers and those exposed to asbestos. The ATBC study used 20mg doses of beta-carotene daily for 5-8 years. A related population study, the Physicians' Health Study, found no adverse effects of 25mg/day of beta-carotene (but also no beneficial effect) but only about 10% of the participants were smokers.
Lycopene is another carotenoid (found in tomatoes, pink grapefruit and watermelon) that has powerful antioxidant activity (about twice as good as beta-carotene) and, like selenium, may be especially effective in prevention of prostate cancer. There are several studies showing a strong association between dietary intake of tomatoes and tomato products (pizza, tomato sauce, and ketchup) and a reduced rate of cancers of the prostate, breast, stomach, lung and colon.
Green tea extract has been shown in a wide variety of studies to provide cancer preventive benefits. Rodent studies show a clear benefit of green tea consumption on rates of skin cancer, while test tube and population studies suggest benefits of green tea on cancers of the stomach, colon, prostate, breast, lung, and liver. The human studies suggest that consumption of 8-10 cups of either green or black tea daily can reduce cancer incidence by 30-40%. Like vitamin C, the polyphenol/catechin compounds are thought to reduce the formation of carcinogenic nitrosamines in the body and may have the additional benefit of providing a direct anti-tumor effect by blocking the activity of specific cancer-specific enzymes. General dosage recommendations for green tea supplements are in the range of 300-600mg/day (3 divided doses of 100-200mg each) ? or about the amount that you might find in 3-6 cups of brewed green or black tea.
Folic acid deficiency is known to increase the risk of cancer of colon, breast and lung ? but it is not known whether dietary supplements may help reduce cancer risk. Because of the other well-known health benefits of folic acids supplements (400mcg/day), such as cardiovascular health (reduced serum homocysteine levels) and reproductive health (reduced risk of neural tube birth defects), this vitamin should be part of almost everybodys daily supplement regimen (as part of a balanced multivitamin that includes B12 + B6).
Flaxseed supplements (the whole seed, not flaxseed oil) contain isoflavone-like compounds known as lignans that may be associated with a reduced risk of certain cancers (flaxseed oil is a good source of omega-3 fatty acids, which have cardio-protective benefits). Like isoflavones from soy or red clover, flaxseed lignans may prevent estrogen from attaching to cells and promoting hormone-sensitive cancers (breast, uterine and prostate)
Shark cartilage is widely touted (mostly on the Internet) as a miracle cure for cancer. While it is quite true that shark cartilage (as well as other sources of cartilage such as chicken, beef and pork) contains compounds that inhibit blood vessel growth (a factor that theoretically could choke-off tumor growth), the test-tube studies that have been done have yet to be completed in human trials.
Supplements during Chemotherapy and Radiation Therapy (??)
A controversial area in the dietary supplement industry right now is that, while we have some evidence that certain dietary supplements may be beneficial in terms of reducing the risk of developing certain forms of cancer (prevention), we are not absolutely sure about whether or not dietary supplements should be recommended during treatment of existing cancer. There are certainly very logical arguments on both sides of the issue ? but neither side has the kind of powerful scientific data to prove their point of view.
Arguing in favor of using dietary supplements during chemotherapy and radiation treatments is the knowledge that antioxidant supplements can help protect healthy cells from the damaging effects of cancer treatment. In many cases, this may result in a better tolerance of the highly toxic cancer treatments and an ultimately more positive health outcome of therapy. Arguing against using dietary supplements during cancer treatment is the theory that cancer cells may also be able to use the protective qualities of some supplements to actually reduce the effectiveness of treatments designed to kill tumors. There is some evidence that tumor cells are able to take up and accumulate high levels of vitamin C ? with the end result that he tumors become highly resistant to treatment via certain therapies such as radiation treatment. Although this may indicate that high dose vitamin C supplements are ill advised during cancer treatment, e simply do not have the scientific evidence for other supplements.
In other studies, there is evidence that some nutritional supplements may actually help chemotherapy work better (primarily from animal and test-tube studies). Some of the drugs used during chemotherapy are known to interfere with vitamin and mineral metabolism. For example, methotrexate interferes with folic acid metabolism ?so cancer patients taking methotrexate should be careful to consume about 400mcg of folate - but no more without guidance of their oncologist (high dose supplements can reduce the effectiveness of the drug). Another chemotherapy drug, Adriamycin can lead to heart damage that appears to be reduced by higher dietary levels of antioxidants, B-complex vitamins and coenzyme Q10. In some cases, vitamin E may enhance the ability of Adriamycin to slow tumor growth (in test-tube studies). Cisplatin, one of the most potent chemotherapy drugs, can deplete the body of magnesium and other nutrients ? yet some of the toxic side effects may be counteracted by supplements designed to promote glutathione synthesis (such as alpha-lipoic acid, selenium and cysteine).
