A 100-year-old drug. A revolutionary new theory of autism. And clinical trial results that made parents cry.
In 2017, Dr. Robert Naviaux at UC San Diego published the most intellectually provocative autism research in a generation — a small clinical trial of suramin in 10 children with ASD.
The result: one intravenous dose improved language, social interaction, and reduced repetitive behaviors. One non-verbal child spoke his first sentence in years.
But the drug is almost secondary to the theory behind it.
The Cell Danger Response — and why it changes everything about how we understand autism:
Every cell in the body has an ancient alarm system. When it detects a threat — toxic metals, infection, nutritional deficiency, gut dysbiosis — it releases ATP into the extracellular space as a danger signal.
Neighboring cells respond by entering a defensive posture: reduced metabolism, reduced communication, reduced development.
This is a brilliant survival mechanism. When it works correctly.
The CDR theory of autism proposes that a significant proportion of children with ASD are locked in a chronic, persistent activation of this cellular alarm — long after the original threat has resolved.
The developing brain is continuously receiving the signal: DANGER. Stay defensive. Do not develop.
And so the neural circuits for social cognition, language, and behavioral flexibility — which require the cell to be in growth and development mode — cannot complete their programming.
The mechanism that sustains it:
Damaged mitochondria continuously leak ATP into the extracellular space.
This ATP binds to purinergic receptors (P2X and P2Y) on neighboring cells — keeping them in permanent CDR activation. The CDR damages more mitochondria.
More mitochondria leak more ATP. The cycle sustains itself.
What suramin does:
Suramin is an antipurinergic drug — it blocks the purinergic receptors that ATP binds to, interrupting the alarm signal.
When the signal stops, cells can finally receive the message: danger is over. Resume normal development.
In a 2024 systematic review of 23 preclinical ASD studies, aberrant purinergic receptor expression and dysregulated ATP signaling were confirmed to contribute directly to behavioral abnormalities, neuroinflammation, and synaptic dysfunction in ASD animal models.
A Phase 2 double-blind, placebo-controlled trial enrolled 52 boys aged 4–15 across 6 sites in South Africa — the most rigorous clinical evaluation of suramin in ASD yet conducted.
PaxMedica is pursuing FDA approval.
Suramin is not yet approved. Do not attempt to obtain it outside clinical trials.
But here is what this means for families today:
The CDR framework validates and explains the mechanism of every biological intervention we already use:
🔴 Zeolite — removes the heavy metals that are one of the most potent CDR activators
🔴 Gut restoration — eliminates the gut dysbiosis generating the continuous gut-derived danger signals
🔴 Mitochondrial support — addresses the damaged mitochondria that are leaking the ATP driving purinergic danger signaling
🔴 Neuroinflammation reduction — breaks the CDR-neuroinflammation reinforcement cycle
🔴 Stem cells — MSCs produce the immunomodulatory factors (IL-10, TGF-β) that suppress CDR-maintaining pro-inflammatory activation
And natural compounds with documented purinergic modulating activity:
🌿 Quercetin — inhibitory activity at P2X3 and P2Y purinergic receptors
🍵 EGCG from matcha — activity at adenosine A2A receptors
🌊 Omega-3 EPA — reduces the prostaglandin danger signals that parallel purinergic CDR activation
We just published the full science article — with 15 references — explaining the complete CDR theory, both clinical trials, the safety context, and how the biology-first protocol maps to the CDR framework.
