- 슬대퇴관절의 부상은 매우 복잡한 상황이 될 수 있으며 치료에 대한 반응이 좌절스러우리만큼 느리다. 다양한 요소들이 슬대퇴관절의 부상에 영향을 미치며, 특히 외상을 일으키지 않는 부상에 있어서 더욱 그러하다. 이 장에서는 가장 흔히 발생되어지는 부상에 대해서 언급되어 질 것이다.
1) 슬개골 탈구와 아탈구
- 슬개골의 불안정성은 남성보다는 여성에서 보다 흔하다. 이러한 현상은 넓은 골반의 크기에 부수적으로 따르는 Q-각의 증가때문으로 추측되어지며 슬개골의 외측벡터의 힘을 증가시킨다. 이러한 증상을 나타낼 수 있는 요소로는 외측 대퇴과상돌기나 후측슬개골 부위가 얕거나, 슬개골의 부적절한 위치상태, 내측광근력이 약해지거나, 특히 빡빡하고 강한 외측구조물과 이러한 현상들이 결합되어질때이다.
- 부상의 원리는 발이 편평하게 놓여 있으면서 무릎관절이 굴곡된 상태에서 대퇴가 외측으로 회전하는 것이다. 만약 슬개골의 아탈구라면 독립적으로 슬개골이 정상위치를 찾지만, 완전 탈구는 독립적으로 정상위치로 돌아가지 못한다. 처음 발생하는 탈구는 정상적인 위치로 돌아가기 힘들고, 통증과 부종도 오래간다.
- 통증과 부종이 회복되고, 재활프로그램이 시작될때 내측광근근력강화와 활동시 슬개골 제어가 반드시 강조되어야 한다. 근력과 슬개골의 제어가 증가될때, 기능성 활동의 속도가 정상적인 슬개골의 제어와함께 가능해질때까지 빠른 속도로 진행될 수 있다.
- 회내가 슬개골 불안정성의 위험을 증가시키므로 주의해야 한다.
- 재활프로그램의 기간은 완전탈구냐, 아탈구냐, 처음 탈구냐, 재발된 탈구냐에 따라 달라진다.
2) 슬개골 주름징후(Plica)
- plica는 50%에서 성인때까지 남아있으며 부수적인 증상으로 활액막염의 원인이 된다. 아래 참조

4) 슬대퇴관절 증후군
- 무릎앞쪽의 통증은 슬개골 연화증, 슬대퇴관절 스트레스 증후군, 슬대퇴관절 동통증후군, 신전근 작용의 잘못된 배열증후군, runner's knee, 슬대퇴관절의 잘못된 배열증후군 등으로 불린다.
- 무릎 신전근에 작용하는 비정상적인 스트레스에 의해 발생되어지는 무릎 앞쪽의 통증, 염증이다. 직접적인 외상에 의해서도 올 수 있지만 부하의 축적에 의해서 흔히 발생되어진다.
- 이러한 요소는 장경인대, 슬건근, 비복근의 뻣뻣함, 내측과근의 약화, 내외측 광근사이 힘의 비평형, 초과되어진 내회전, Q각의 증가, 무릎의 과신전, 슬개골배열의 이상 등이다.

- 만약 슬건근, 비복근, 장경니대 혹은 외측 결합조직 구조물이 뻣뻣하다면 이러한 것들이 무릎관절에 조화되지 않은 힘을 작용할 수 있다.
- 뻣뻣한 슬건근은 활동시 무릎굴곡을 하는데 더 많이 필요가 되어지고 슬개골전방에 압축력을 증가시킨다. 걷는 동안 증가되어진 무릎 굴곡의 필요성은 차례로 발가락을 올리기 위해 저측굴곡의 증가가 필요할 것이다.

슬개대퇴 증후군 (patellofemoral pain syndrome)

슬개대퇴통증 증후군이란 슬개골, 슬개-대퇴관절, 슬개골하 인대, 내, 외측 슬개인대, 대퇴사두근들의 협동 작용에 이상으로 슬관절 굴곡/신전시 슬개골이 대퇴관절위에서 움직일 때 슬개인대 또는 슬개골하 연골에 무리가 가서 나타나는 통증을 말한다.
전슬 통증증후군(anterior knee pain syndrome), 또는 과사용 증후군 또는 런너의 무릎(runner's knee)라고도 하며 달리기 손상중 가장 흔한 손상중 하나이다.
대부분의 환자는 외견상 부종이나 변색을 발견할 수 없고 간단한 진찰, 단순방사선 촬영에
서도 특별한 소견을 발견할 수 없으므로 거의 모든 환자가 여러 병원을 찾아다니면서 인대
에 무리가 갔다, 관절염초기, 점액낭염, 슬개골하건초염, 대퇴근막통증후군등 여러 가지 진단명을 듣게 되는데 실제 앞서의 여러 질환과 감별이 어렵고 또한 감별이 필요하다.
이 질환은 10대의 성장기 아이와 40대 이후에 많이 발생 하는데
성장기에는 발달 단계상 슬외반(Valgus)가 나타나 대퇴-슬개인대 각도가 증가하고 빠른
성장으로 골격과 근육의 불균형이 초래되어 특히 외측 광근의 근력이 증가되어 슬개골이
외측으로 당겨져서 연골의 연화와 통증을 유발하는데 대개 성장이 멈추면서 호전된다.
40대이후에는 우리 몸의 모든 연골이 퇴화/퇴행이 일어나는데 슬개골 연골도 비교적 빠르게
퇴행되어 연골 연화증 심한 경우는 관절염으로 진행된다.
슬개골은 다리에서 체중부하를 받지 않는 유일한 뼈이다. 슬개골은 무릎 관절 앞에 있으며 아래쪽으로는 경골에, 윗쪽은 사두근과 슬개골 인대에 의해 각각 연결되어 있어 무릎을 보호하고 있다.
슬개골에 대한 압력은 계단을 오를때는 체중의 2배가 되지만 계단을 내려 갈때는 체중의 7배가 된다고 한다.
슬개골은 무릎을 구부리거나 펼 때 대퇴골의 접촉부위( intercondylar groove)를 따라 움직인다. 슬개골은 위, 아래 방향뿐만 아니라 관절면에 따라 기울거나 회전 할수있어 여러 관절면과 접촉하게 된다.
정상적으로 움직일수 있는 통로를 따라 이동에 장애를 주는 어떤 요인이 있으면 슬개 골 아래에 염증이나 통증을 일으킨다.
슬개골 대퇴 증후군은 무릎 앞쪽 통증의 흔한 원인중 하나이다.
슬개골의 내측 균형인자인 광내사근(vastus medialis oblique)이 외측균형인자인 장경인대, 외측 횡단인대, 광외사근보다 상대적으로 더 약할때 슬개골이 외측으로 편향되는데, 특히 슬관절의 신전위 마지막 20 ° 에서 잘 발생한다고 한다.
증상은 무릎을 구부리면 악화되는데 이는 슬개골과 대퇴골의 관절면의 압력이 증가하게되고 손상을 부하되는 부위에 과도하게 긴장된 힘은 통증을 유발하게 된다.
심하지 않는 경우에서는 슬개골 아래의 면에 단순히 염증만 생길수 있지만 더 심해지면 연골이 손상되거나 관절염으로 진행할수 있다.
상해나 과도한 사용은 두껍고 매끄러운 관절 표면이 연해지고 거칠어 지며 불안해지게 된다. 만약 슬개골이 움직이는 통로가 잘못되면 슬개골의 불안정성이 생겨 아탈구나 실제 탈구가 발생할수 있다.
슬개대퇴 증후군과 슬개골연골 연화증의 다른점은 대퇴골과 슬개골사이에서 통증을 일으킬수 있는 전반적 경우의 진단을 슬개대퇴 증후군이라고 하며, 슬개골과 대퇴골 사이의 연골이 병리적 변화와 연화가 생겨서 통증이 생기면 슬개골 연골 연화증이란 진단명을 사용한다. 슬개골 연골연화증은 무릎 앞쪽의 통증을 일으키는 슬개대퇴 증후군중에서 관절경을 시행 하여 연골의 병리 조직적인 진단이 있는 경우라고 할수 있다. 즉, 슬개대퇴 증후군은 슬개골연골 연화증의 진단전 단계의 명칭이라 할수 있다.
suprapatellar (SPP) (above the patella or kneecap)
infrapatellar (IPP) (below the patella), sometimes called the 'ligamentum mucosum' or 'filmy ligament' as it mimics the cruciate ligament inside the knee, but is not a ligament at all.
medial (MPP) (on the inner aspect of the knee). It may form a shelf-like band and is then referred to as a 'medial shelf'.
In the illustration above, looking at the knee bones of the right knee from its inner aspect, you can see the relationship of the media and suprapatellar plica. Here the leg is straight.
When the knee is bent, the plicae taughten, and a medial plica can snap abruptly over the medial femoral condyle, while the suprapatellar plica may be nipped between condyle and the quadriceps tendon.
The medial plica can also become nipped between patella and condyle, causing momentary pain and 'giving way'.
A plica is a fold of the soft inner lining of the knee joint 'the 'synovium'. This can also be considered to be related to the extensor mechanism (see menu), although indirectly.
Plicae (plural) are remnants of structures which existed in the foetus (in the womb) and which subsequently fell away to a lesser or greater extent as the knee developed. In some people they are still significant stuctures and in other people they may be completely absent.

Plica Syndrome
Plicae are important to know about as they way in which they can cause problems is often misunderstood.
First of all, one must know that many cause no problems at all. But often the plica fold can get injured and swollen and then get nipped between the bones of the patella and femur (thighbone), or between the patella and the undersurface of the quads muscle in the case of the SPP. This problem, with its attendant symptoms, is known as the 'plica syndrome'.
One of the problems with diagnosis is that surgeons do not often use what is called a 'superior portal' - looking into the knee cavity from above - and the mechanism of pain production by a suprapatellar plica and often a medial plica, too, can be virtually invisible unless one does.
Symptoms of the plica syndrome will depend upon which plica is causing the problem. A problematic medial plica may have the following associated with it:
pain along the inner aspect of the patella
a string-like cord under the skin in this area
intermittent 'catching' of the joint at a particular point in extension
occasional 'pseudo'-locking, ie the joint seems to lock but can be easily unlocked
stiffness and pain after sitting still for a long time, eg at a movie
occasional giving-way
A damaged plica may settle on its own, or respond to non-surgical measures such as physiotherapy or injections. However, sometimes the nipping continues and the joint cartilage becomes damaged and arthritis may set off. In this case surgery to remove the plica is the best option.
