국문 초록 (Abstract)
본 논문은 ADHD로 진단을 받은 아동의 주 양육자인 어머니에게 있어 자신의 아이가 ADHD로 진단을 받은 시점에서부터 아이에게 약물치료를 선택하게 된 계기와 그 과정에서의 ‘마음갉기’ 경험을 심층적으로 탐색하였다. 이를 통해 ADHD에 대한 기존 및 현재의 병리학적 처우에 대한 비판적 식견을 얻음으로써 새로운 ADHD관과 그에 따른 사회복지학적 제언을 끌어내고자 하였다.
본 연구는 질적 사례연구로 Mad Studuies의 분석 틀을 기반으로 하여 ADHD 진단을 받은 아동의 약물치료에 따른 어머니의 ‘마음갉기’ 경험은 어떠한가? 라는 연구 질문에 대해 다음과 같은 연구 결과를 도출하였다. 먼저 마음갉기의 시작, 마음갉기의 여정, 마음갉기의 종착의 3단계로 구분하여 44개의 의미 있는 개념을 분류한 후, 17개의 하위범주를 도출하였고, 도출된 하위범주를 추상화하여 ‘ADHD 증상의 발현과 부정적 정서’, ‘치료과정의 애로’, ‘약물치료에 대한 복잡한 생각’ ‘병원치료에 대한 양가성’, ‘ADHD에 대한 편견과 차별’, ‘사방팔방으로 막힌 삶’, ‘치료에 대한 관점 변화’, ‘ADHD에 대한 인식 변화’라는 8개의 범주를 도출하였다. 분석 결과를 근거로 아래와 같은 제언을 하였다.
ADHD의 진단방식에 있어 전문가의 창의적인 진단이 요구되며, 정신건강의학과 진단 및 처방에 있어 낙인효과를 방지할 수 있도록 개인 정보의 보호가 절실하다. 또한, ADHD 진단을 받은 아동의 약물치료의 실체에 관한 엄밀한 고찰과 비판적 성찰, 그리고 부모의 정서적 고통에 대한 사회적 지지와 부모교육이 필요하다. 무엇보다도 ADHD 진단 아동에 대한 긍정적인 면을 찾는 시각이 불가결하다. 더불어 다양성의 일면으로 ADHD의 존중과 인간의 다양한 측면의 하나로 인정해야 함도 중요하다.
본 연구에서 밝힌 어머니의 마음갉기 경험은 정신장애 영역에서의 장애학적 시각과 정확히 맥을 같이 한다. 이것이 본 연구와 관련되는 지점은 ADHD에 대한 인식변화의 깨달음은 Mad Studies적 시각으로 표현되는 장애 학적 패러다임으로의 전환을 시사하는 것이다.
본 연구에 따른 한계점과 후속 연구는 다음과 같다. 먼저 연구 참여자들은 연구자와 지속적인 관계를 유지해 온 어머니들로 연구내용에 대해 충분한 설명을 드렸지만, 연구자가 의도하는 바를 인식하고 인터뷰 시 호의적인 응답을 했을 가능성은 존재하며, 연구 결과에 미치는 영향도 있을 수 있다.
또한 ADHD 진단을 받은 아동의 약물치료를 경험한 주 양육자인 어머니의 경험에 관해 심층적으로 고찰하였으나, 질적 연구의 특성상 소수의 연구 참여자로 인해 일반화에의 제약이 따른다는 것이다.
향후에는 근래의 한 부모 가족화 현상 등을 고려하여 주 양육자로 아버지의 경험을 살펴보기 위한 추가 사례연구가 필요할 것으로 생각한다. 또한 ADHD 진단을 받은 아동의 약물치료를 경험한 연구로써 자녀가 ADHD 진단을 받았음에도 약물치료나 비약물 치료 등을 선택하지 않은 양육자의 경험에 관한 사례연구도 필요할 것이다.
새로운 ADHD 관에 의한 당사자 연구가 미흡한 현상으로부터는 ADHD 진단을 받은 아동에 대한 생애 주기적 관점에 따른 다양한 연령대의 당사자 관점을 탐색할 것도 요구된다. 이와 관련한 세부 연구주제로는 정신의학 서비스 이용자의 치료 장기화, 서비스의 폐해와 남용, 의료인과 ADHD 환자 간의 권력 관계 등에 관한 심층적이면서도 철학적인 고찰이 필요할 것이다.

This paper aimed to promote a new ADHD view and social welfare suggestions by in-depth exploration of the opportunity for a mother, the main caregiver of child diagnosed with ADHD, to obtain critical knowledge of the existing and current pathological treatment of ADHD. This study was a qualitative case study, and what is the mother's experience of "reading mind" according to drug treatment of children diagnosed with ADHD based on the analysis framework of Mad Studies?
The research question was derived as follows. After classifying 44 meaningful concepts into three stages: the beginning of mind-reading, the journey of mind-reading, and the end of mind-reading, 17 subcategories were derived, and abstracted the derived subcategories to "expression and negative emotions," "difficulties in the treatment process," "positive thoughts on hospital treatment," and "biases." Based on the derived results, the following suggestions were made.
Creative diagnosis by experts was required in the diagnostic method of ADHD, and personal information protection is urgently needed to prevent stigma effects in diagnosing and prescribing mental health medicine. In addition, it was necessary to strictly consider and critically reflect on the reality of drug treatment in children diagnosed with ADHD, and social support for emotional pain of parents and parental education were needed. Above all, it was necessary to find a positive aspect of children diagnosed with ADHD. As one aspect of diversity, it was important to respect ADHD and to recognize various human aspects. The mother's mind-reading experience revealed in this study is exactly in line with the disability perspective in the area of mental disorder, while the realization of the change in perception of ADHD suggested a shift to a disability paradigm expressed in Mad Studies perspective. The limitations and follow-up studies of this study were mothers who have maintained a continuous relationship with the researcher, but there was a possibility that the researcher recognized their intentions and responded favorably during the interview, and might have an effect on the research results. The mother, who was the main caregiver who experienced drug treatment in children diagnosed with ADHD, was considered, but due to the nature of the qualitative study, there were restrictions on generalization due to a small number of study participants. In addition, additional case studies were needed to examine the father's experience as the main caregiver. In addition, as a study that experienced drug treatment in children diagnosed with ADHD, a case study on the experience of caregivers who did not choose medication or non-drug treatment even though their children were diagnosed with ADHD would be needed.
Due to the lack of party research by the new ADHD view, it was necessary to explore the party's perspectives on various age groups according to the life cycle perspective of children diagnosed with ADHD. In-depth and philosophical considerations were needed on the prolonged treatment of psychiatric service users, the harmful effects and abuse of services, and the power relationship between medical personnel and ADHD patients.
