실제 gas 교환에는 관여하지 않으며 혈관도 분포하지 않는, 단지 공기의 전달 통로로 공기유입의 첫 통로부터 trachea, bronchus, 1차 bronchiole, 2차 bronchiole와 terminal bronchiole까지 |

★respiratory airway |
|
|
|
실제 gas 교환에 관여하는 영역으로, respiratory bronchiole 및 alveolar sac |
|
해부적 측면에서 upper respiratory system과 lower respiratory system 으로 구분 |
|
|
1) Upper respiratory system : mouth, nasal cavity, pharynx, larynx |
|
|
2) Lower respiratory system : trachea, bronchi, bronchioles |

(코가 막혔을 때도 구강호흡이 가능하다. 그러나, 구강은 비강보다 air conditioning 기능이 약하다.)

② Pharynx (throat) |
|
|

비강과 구강이 만나는 곳으로 음식물과 공기가 함께 지나는 통로 |
|
|

trachea와 esophagus로 분리 |
|
|
|

|
|
③ Larynx ( voice box ) |
|
|
- |
vocal fold : elastic tissue로 이루어진 2개의 band, larynx 입구에 존재 |
|
|
- |
laryngeal muscle에 의해 다양한 모양으로 인장되거나 위치한다. |
|
|
- |
음식을 삼키는 동안에는 서로 tight apposition을 형성해서 trachea 입구를 차단한다. |

|
|
|
2) Lower respiratory system : |
|
|
|
|
|

ciliated epithelium → secretion of mucus & dilute saline solution |
|
|
- |
cilia → mucus escalation |

① Trachea |
|
|
- |
C자모양의 cartilaginous ring --> 항상 open되어 있으며, rigid tube. | |
|
|
|
|
|
|
② Bronchi |
|
|
- |
내부로 내려갈수록 점점 작아지며, cartilaginous ring은 있으나 smooth muscle이 없으며 항상 open되어 있고 직경조절이 불가능하다. | |
|
|
- |
그 끝은 left와 right로 나뉘어 각각의 lung으로 연결된다. | |
|
|
|
|
|
|
③ Bronchiole |
|
|
- |
각각의 lung내부에 highly branched, more numerous tube로 되어 있다. |
- |
trachea에서 bronchiole 쪽의 내부로 갈수록 직경 감소 및 가지의 분지 증가, cartilaginous ring이 줄어들어 없어지고, smooth muscle은 늘어나며, 이 smooth muscle은 autonomic nervous system이나 local chemical에 의해 자극되어 air passing amount를 조절한다. | |

④ Alveoli |
|
|

clusters of thin-walled, inflatable, grapelike sacs at terminal branches of the airway |

pulmonary capillary에 의해 둘러싸여 있음 |

only gas exchange site between air and blood (diffusion에 의한 gas exchange가 용이한 구조) | |

|
|
|
i) Single layer of flattened type I alveolar cell |
|
|
capillary 내벽과 interstitial space를 거쳐 alveolar 내벽까지 거리 → 0.5um (very thin) |
|
ii) Tremendous surface area for exchange : |
|
|
the alveolar air-blood interface |
|
iii) Pulmonary surfactant : |
|
|
type I cell과 더불어 alveolar epithelium을 구성하고 있는 type II alveolar cell이 분비하는 phospholipoprotein complex |

alveolar macrophage : air sac의 lumen에서 phagocytosis 담당. |

pore of Kohn : airflow between adjacent alveoli (collateral ventilation) | | |
