1) Pacinian corpuscle
location - fibrous layer of capsule
description - single terminal with clusters of five
sensitivity - high frequency vibration(>60Hz), acceleration, high velocity
; hemohydraulic, transient events, rapid contraction
distribution - distal joint ; laryngeal & middle ear joint
functional classification - very rapid adapting(RA) ; very low threshold
parent axon - Group Ⅱ ; 8-12㎛, 49㎳; terminal branch - 3-5㎛
2) Golgi-Mazzoni corpuscle
location - inner surface joint capsule between fibrous layer & subcapsular tissue
description - multiple terminal within thin encapsulation
sensitivity - compression of joint capsule
distribution - knee joint & other joint
functional classification - slowly adapting(SA); low mechanical threshold
parent axon - Group Ⅱ-Ⅲ ; 5-8㎛, 30㎳
3) Ruffini ending
location - fibrous layer of capsule
description - spray-type terminal within encapsulation
sensitivity - stretching to long axis within capsular plane, speed stretch,
; joint position change, intracapsular fluid pressure change
distribution - proximal joint
functional classification - slowly adapting(SA) ; low mechanical threshold
parent axon - GroupⅠ-Ⅱ; 13-17㎛ or 8-12㎛, 51㎳
4) Golgi ligament ending
location - extrinsic & intrinsic ligament
description - thick encapsulation, profuse branch
sensitivity - stretching ligament
distribution - most joint except cervical vertebral, laryngeal & ossicular ligament
functional classification - slowly adapting(SA); low to high threshold
parent axon - GroupⅠ; 13-17㎛ ; 51㎳
5) Free nerve ending(non-nociceptive & nociceptive)
location - fibrous capsule, ligament, subsynovial capsule, fat pad, synovium
description - thin, bare nerve ending
sensitivity - some type - nonnoxious mechanical stress
others type - noxious stress & biochemical stimulation
distribution - all joint; depend on joint component; high density at joint ligament
functional classification - slowly adapting(SA); low to high threshold
parent axon - Group Ⅲ-Ⅳ(2-5㎛, <2㎛; 2.5-20㎳)
2. 분류법에 따른 분류
관절의 기계적 수용기: TypeⅠ, TypeⅡ, TypeⅢ, TypeⅣ.
1) TypeⅠ
Location
: 슬관절의 posterior oblique ligament, collateral ligament과 같은 인대나 관절낭의 섬유층에서 주로 발견된다.
: 즉 관절낭의 얕은 층
Distrobution : proximal joint와 cervicle vetebral에서 많이 분포해 있다.
- 즉 hip, shoulder, neck
: anterior & posterior aspect에 많이 분포해 있다.
myelinated parent axon에 2-6개의 cluster가 달려 있다.
: hip, shoulder, neck의 tonic fiber
Morphology : Ruffini ending과 비슷하기 때문에, Ruffini-like라 불린다.
Function : static과 dynamic physiologic functions에 관여한다.
resting discharge rate : 1-20pps
action시에는 discharge rate 가 변한다.
inhibit pain
Behavior : Postural & kinesthetic sensation
1분 정도까지 작용
middle range에서는 작용하지 않음
Threshold : low
Adapting : slow
2) TypeⅡ
Location : intraatricular & extraarticular fat pad와 관절낭의 섬유층
: 관절낭의 심부층
Distrobution : distal joint(손이나 발, TMJ, lumbar)
: 관절낭의 medial & lateral part
myelinated parent axon에 1에서 5개 정도의 receptor가 달려 있다.
: 손, 발, 턱, 요부의 phasic fiber
Morphology : pacinian - like
Function : dynamic physiologic functions에 관여한다.
inhibit pain
Behavior : dynmic sensation, vibration에 의해 작용함
Threshold : low
Adapting : rapid
3) TypeⅢ
Location : collateral ligament, thoracolumbar의 facet joint, 척추의 longitudinal & interspinal
ligament
cervicle에는 발견되지 않음
Distrobution : ligament의 deep & superficial층과, lumbar spine 관절낭의 superficial층
myelinated parent axon에 한개의 corpuscle이 존재
Morphology : Golgi tendon과 비슷한 형태인 Golgi ligament ending
Function : static과 dynamic physiologic functions에 관여한다.
resting discharge rate : 1-20pps
action시에는 discharge rate 가 변한다.
not inhibit pain
Behavior : dynamic sensation , ROM의 end range에서 stretching에 반응함
: 움직이지 않는 관절에서는 반응하지 않음
Threshold : high
Adapting : slowly
4) TypeⅣ
Location : capsule, ligament, fat pads, blood vessels, anterior dura mater, connective tissue
: 그러나 muscle, nerve, brain blood vessels, cartilage 에는 존재하지 않음.
Morphology : free nerve ending
Function : pain provoking
Behavior : pain receptor로서 44.8°이상의 온도, 염증, 과도한 신장에 의해 작용할 때
tonic reflexogenic effect가 있슴
Threshold : high - 모든 사람은 같은 역치를 가지고 있다. 그러나 인내성은 다르다.
Adapting : slowly
speed :A-δ=5-15m/s, B섬유 = 3-14m/s, C 섬유 = 0.02-2m/s
요약
TypeⅠ = Ruffini ending - proximal joint, cervicle vertebral joint : superficial layer

TypeⅢ = Golgi-Mazzoni corpuscle
TypeⅣ = free nerve ending
모든 관절은 primary nerve(pure articular nerves)와 accessory nerve(대개 muscle nerve에서 분지되는 nerve)에 의해 신경지배를 받는다.
예: knee joint의 innervation은 3개의 articular nerve에 의해 신경지배된다
: posterior articular nerve, medial articular nerve, lateral articular nerve와 quadriceps, satorius, gastrocnemius의 nerve에서 분지하는 small intramuscular articular nerve도 많이 있다. posterior articular nerve는 knee joint의 가장 큰 신경으로, popliteal fossa 바로 밑에서 posterior tibial nerve에서 분지된다.
: medial articular nerve는 obturator nerve나 saphenous nerve에서 분지하여 thigh의 anteromedial region에서 시작된다.
lateral articular nerve는 common peroneal nerve에서 분지하지만, 존재하지 않는 경우도 많다.
ligament에도 nerve이 풍부하게 존재한다. 예를 들어, ACL의 총체적중 1.0-2.5%가 nerve로 되어 있다.
joint receptor(Ⅰ, Ⅱ)는 joint capsule과 ligament에서 변형이 일어날 때만 반응하는 것으로 생각된다. joint receptor(Ⅰ, Ⅱ)는 관절가동범위의 모든 범위에서 일어나는 것이 아니라, 최대 굴곡지점이나 최대 신전지점의 특정 지점에서 활성화된다. 그러나 최대 굴곡과 최대 신전사이의 차이 정도는 명확하지 않다.
2. 신경섬유
전통적으로, sensory fibers는 그 섬유의 직경과 신경전달속도를 기준으로 4 범주로 분류한다.
GroupⅠ은 가장 큰 직경(10-18㎛)을 가지고 있으며, 전달속도는 60m/s이상이다.
GroupⅡ은 5-12㎛의 직경으로 되어 있고, 전달속도는 20-70m/s 정도이다.
GroupⅢ은 1-7㎛의 직경으로, 전달속도는 2.5-30m/s 정도이다.
GroupⅣ은 전달속도가 2.5m/s 이하인 섬유
Group 형은 일차 구심성 신경원(primary afferent neuron)을 의미하고,
Type 형(Freeman과 Wike에 의해 명명)은 receptor(sensory nerve ending)을 의미하는 것이다.
다시 말해, GroupⅠ 과 TypeⅠ 은 다른 것이다.
그리고 TypeⅠ의 관절수용기가 GroupⅠ 의 신경섬유에 연결되는 것도 아니다.
그리고 group 형을 다음과 같이 분류하기도 한다:
GroupⅠ = Aα(Ⅰa fiber와 Ⅰb fiber)
GroupⅡ = Aαfiber
GroupⅢ = Aδ fiber
GroupⅣ = C fiber
3. 기계적 수용기와 관련된 척수로
관절수용기와 관련된 척수로는 여러 척수로중 고유수용기가 작용하는 척수로에 해당한다. 여기에는 사람이 의식해서 알 수 있는 것(Cerebral Cortex로 가는 경로)과 무의식적으로 작용하는 척수로(Cerebellum로 가는 것)가 있는데, 그들이 작용하는 경로는 다음과 같다:
