Tinnitus Otosound Products라는 회사와 UC San Diego가 함께 이명치료를 위하여 개발 중인 치료가 있어 알려 드립니다. 각 개인의 이명소리를 Computer Program으로 복사해 MP3에 옮겨서 들으면 이명소리가 현저히 줄어들거나 아주 없어지기도 한다고 합니다. 현재 임상중이며 FDA에도 허가 출원중이랍니다. 빠르면 앞으로 8-9 개월 시판될 예정이라는데 기대해 봅니다. 혹 학술적으로 흥미가있는 분을 위해 원문을 아래에 첨부합니다. 원문의 기본적인 내용은 이명의 원인을 (소리) 찾아 그소리로 치료를 한다는 것입니다. 대부분 이명은 (다 그렇지는 않지만) 청각기 계통의 장애로 어떠한 주파수의 음을 듣지 못 할 경우 뇌를 통해서 그 주파수의 소리를 증폭해 느끼기 때문에 이명이 됩니다. 이런 원리를 이용해 그 듣지 못하는 주파수의 음을 기계로 측정해 그 주파수의 음을 만들어 계속해서 듣게되면 치료가 된다는 이론입니다. 치료 기간이 길지않고 (얼마라고는 명시가 안되어 있슴) 효과가 탁월 하다고 합니다. 이명 치료를 효과적으로 하기 위해서는 환자의 이명소리 보다 조금 작게 치료기의 소리를 맞추는데 한 환자의 경우 치료 도중에 현저하게 이명 소리가 적어져서 그 적은 소리에 맞추기 위해 다시 치료기를 제작하기도 했답니다. 본래의 치료기로는 더 이상 적은 소리를 낼 수가 없어서였기 때문이랍니다 (원문 마지막 구절 참조). 이명의 완치는 아니지만 느끼지 못 할 정도로 작게 들리고 일상 생활에 커다란 지장이 없다면 우리에게는 많은 도움이 되리라 봅니다.
For many years, investigators have been focusing on permanent changes in the peripheral auditory system as being primarily responsible for the etiology of tinnitus. However, recent research suggests that while tinnitus may be triggered by events in the periphery, the mechanisms that transform tinnitus into a persistent and debilitating condition are located in the central nervous system. This implies that therapy should be directed at central auditory function to treat tinnitus. This is precisely the direction taken by Tinnitus Otosound Products.
Many previous efforts aimed at treating tinnitus have targeted the peripheral auditory system. In contrast, Tinnitus Otosound Products uses Customized Sound Therapy to address the underlying pathology in the central nervous system.
Tinnitus patients unintentionally condition themselves through negative reinforcement. It has been demonstrated, for instance, that cortical representation of tones associated with unpleasant sensations is enlarged [Gonzalez-Lima and Scheich, 1986]. It has also been shown that neurons in the primary auditory cortex of gerbils change their response characteristics to tones coupled with aversive unconditioned stimuli [Ohl and Scheich, 1996].
A computational model of tinnitus based on extensive animal studies further elucidates that the limbic system is a necessary component for stabilizing the tinnitus perception. This model also explains how a peripheral hearing deficit leading to decreased auditory input can give rise to a specific tinnitus pitch.
From the given body of evidence, Tinnitus Otosound Products investigators have deduced that if the tinnitus sound itself is used as a habituation stimulus, the tinnitus should disappear. Thus, CST was developed to precisely replicate a patient’s tinnitus experience with a custom generated frequency-specific sound delivered by a portable digital sound device. By selectively and continuously stimulating the same population of neurons affected by the tinnitus over a period of time with CST, corrective habituation occurs rapidly and efficiently. The method for sound replication follows a successive approximation procedure pursuant to a unique algorithm.
• UCSD scientists have also discovered a reliable EEG (electroencephalogram) correlate for tinnitus that they have named the N100 EEG index (N100). N100 is a diagnostic technique that uniquely provides an objective measure of a patient’s tinnitus experience. Previously, a method to quantitatively measure the degree of a patient’s tinnitus was not available. Not only is N100 a potentially useful tool in assessing the severity of a patient’s tinnitus, but it may eventually serve as a means to dynamically modulate CST to improve its efficacy. Therefore, N100 feedback may be utilized in building a next-generation CST device that could lead to even more rapid and effective treatment. Tinnitus Otosound Products filed a patent for N100, A tinnitus patient arrives at a clinic offering CST treatment. The patient is given headphones connected to an interactive “sound matching station” that presents a variety of sounds and prompts the patient for feedback. Over one hour a period, the test administrator uses the station to successfully replicate the exact tinnitus profile of the patient.
Therefore, N100 feedback may be utilized in building a next-generation CST device that could lead to even more rapid and effective treatment. Tinnitus Otosound Products filed a patent for N100, which is expected to be approved shortly.
Each person’s tinnitus experience is unique and CST is programmed individually for each patient. Below is a spectrogram of a typical Tinnitus Otosound Products CST tinnitus habituation stimulus, showing two closely-spaced narrow band noises centered at 2800 Hz and 3225 Hz, and a very narrow band noise centered at 7417 Hz which is almost 40 dB stronger than the first two.
The unique sound file is then downloaded into a portable sound player device that the patient wears as often as comfortable over a period of several weeks.
• Over time, the vast majority of patients experience a notable reduction in the intensity of their tinnitus and in some cases a reversal of the disease.
In order for effective habituation to occur, the customized sound should be audible, but not loud enough to mask the tinnitus sound itself. Therefore, the volume settings that tinnitus subjects use on their customized sound generator give an estimate of the intensity of the tinnitus. The volume settings that UCSD tinnitus subjects use during pilot experiments markedly decreased over time, in one case so much that the subject exhausted the available intensity setting on sound generator and the stimulus need to be re-synthesized at a lower intensity. The efficacious effect of CST have been very well substantiated in human subjects.

apostle2002님 기존의 소리발생기는 각 개인의 이명 특성과 관계없이 고정된 주파수의 소리를 만들어서 이명에 적응 하는 치료를하는 반면에 새로운 것은 각자의 이명 특성에 맞추어 소리를 특수 제작해서 치료를 하는 것입니다. 이러한 이유로 치료 효과가 훨씬 높은 것으로 나타나고 있습니다. 제가 알고 있기로는 대부분의 경우 이명 자체의 소리는 변화가 없고 우리의 느낌에 따라 소리가 변하는 것처럼 인식을 하는데 여기 첨부된 원문 내용에서는 이명 소리가 줄어들기도 하고 또 어떤 경우에는 완전히 없어지기도 한답니다. 언젠가는 우리 모두 함께 웃을 날이 있으리라 굳게 믿습니다.
