PD진전과 본태성진전과는 비슷한 점이 많이 있는 것 같습니다.
저는 처음에는 떠는 것이 전혀 없는 상태에서 시작했고, 몇 년후에 약간의 떠는 것이 생겼는데 그것이 pd의 진전과 관계가 있는지 아닌지에 대한 정확한 진단을 받지 못하고 있습니다.
흔히 알고 있는 바에 따른 pd의 휴식성진전의 모습이 아니라는 것인데, E-T(본태성진전, 수전증?)은 집안의 내력과도 상관이 있는 것이라는데 주변의 그런 일 없었고, 약기운 떨어지면 현저히 나타나는 바로 제 떨림은 pd와 상관이 있다는 것이 제 생각입니다.
저도 그렇고 많은 분들이 자신의 떨림이 pd와 관계가 있는지에 대해 궁금해 하시는데, 저는 떨림에 대하여는 겪은 바가 적어서 제 생각이나 경험은 부족합니다.
그래서 E-T(본태성진전)과 pd의 진전에 대한 차이를 한 번 찾아 놓은 것을 적어 올립니다.
이 두 가지를 비교해서 얻은 차이는,
PD에서
*휴식성진전은 PD에서만 나타난다.
*보통 어느 한 쪽(오른쪽 왼쪽중)에 강하게 나타난다.
*휴식성진전을 포함하여 PD진전의 20%는 팔을 뻗을때 (extend their arms:postural tremor)와 움직일때(move their arms:action tremor)나타난다.
E-T에서의 진전은
*수면시 나타나지 않는다.
*손떠는 것이 가장 흔히 나타나며, 주로는 양손에 나타난다. 그러나 10-15%는 잘쓰는 쪽 손에(오른손잡이. 왼손잡이) 나타난다.
*PD가 휴식성진전인 것에 반하여 이 E-T는 자세성진전(postural tremor-본인 스스로 힘을 써서 어떤 행동을 하고자 할때, 중력에 반대하는 힘을 쓰는 일을 할 때 생기는 떨림)이나 의도적진전(intention tremor-물건을 집거나 할 때 생기는 떨림) 또는 두가지 다로 나타난다.
*떠는 모습이 위,아래(yes-yes로 E-T의 1/4)로 떨거나, 옆(no-no로 E-T의 3/4)으로 떤다.
*진행이 느려서 대부분의 경우 심각한 상태로까지 발전하지는 않는다.
이와 같이 대체적으로는 PD는 휴식성진전으로, E-T는 움직일때로 나타난다로 말하여지나 반드시 그런것만은 아닌것 같으며.
또 Pd와 구별되어지는 PD-plus의 경우는 또 다른 증세를 갖고 있으니
떨림 하나만으로 PD이냐 아니냐등을 결정짓기 어려우며, 다른 여러 증상을 포함하여 생각되어져야하며, PD진단시에 흔히 하는 레보도파에 대한 반응도 그러한 의미에서 하나의 방법으로 쓰고 있는것이라고 생각합니다.
이러하기에 환자의 자각증세를 관찰, 메모하시어서 의사와 의논하시는 것이 좋지 않겠나-하는 생각입니다.
PD 진전
Tremor which the patient feels at rest with the arms relaxed and not moving. The
resting tremor of PD usually starts asymmetrically. If a resting tremor is present, and marked, and is more on one side than the other, it is virtually specific for PD. A resting tremor is rarely present in the PD-like disorders.
20% of PD patients, usually in addition to a resting tremor, have a tremor when they extend their arms (called a postural or sustention tremor) or when they move their arms (called an action of kinetic tremor). And some patients have both a postural and action tremor. In half of patients with PD and a sustention tremor the tremor appears before the resting tremor and may be confused with a condition called Essential Tremor. Essential Tremor is perhaps 20 times more common than PD. In perhaps 10 % the tremor is sufficiently severe to require treatment. Rarely, ET becomes PD.
Essential tremor: 본태성진전
is considered the most common neurologic movement disorder. It may affect as many as 10 million people in the United States and be 20 times more prevalent than Parkinson's disease. ET is a chronic condition characterized by involuntary, rhythmic tremor of a body part, most typically the hands and arms. In most patients, ET is considered a slowly progressive disorder and, in some patients, may eventually involve the head, voice, tongue (with associated dysarthria), legs, and trunk. However, in many people, the disease may be relatively non-progressive and the tremor may be mild throughout life.
Hand tremor is the most common form of essential tremor. It is typically biphasic and involves agonist and antagonist muscles. It is usually present in both hands (bilateral); however, in about 10% to 15% of patients, tremor is first noted in the dominant hand. The frequency of the tremor is between 4 to 12 Hz.
Tremor may be most visible while the patient is voluntarily maintaining a fixed position against gravity (e.g., outstretched arms, etc.). This type of tremor, termed a "postural tremor," is a major component of classic ET. The tremor is apparent while holding the body part in a fixed position. In some patients, the tremor may worsen upon performance of self-directed tasks or goal-directed movements. This component of essential tremor is termed as an "intention tremor" meaning that it is present with targeted actions, a form of "kinetic tremor". ET sometimes results in what is referred to as an "internal tremor." Patients most often describe this feeling as a general "shakiness" or a vibrating sensation in the body. All tremors usually disappear during sleep. Unlike the resting tremor associated with Parkinson's disease during which muscles are not voluntarily activated, the symptoms of essential tremor are either absent or minimal during periods of rest.
첫댓글 파킨슨의 진전은 휴식성이고 운동을 하면 없어진다라는 설명에다 대입하면 무릎는 휴식자리 이고 탁자, 책상위는 일을 하는거라서인지... 그러니까 수전증쪽이 아닌가?싶고....무척 예민하신것같으네요
