Excellencies, dear colleagues and friends,
Good morning to all of you, and a very happy New Year.
The dawning of a new year gives us the opportunity to look forward with hope, and to look back on the year that has been.
And as I look back on 2023, it is with mixed emotions.
It was a year to celebrate, as we marked our 75th anniversary, and reflected on the many improvements in human health that we have been part of.
There were also many achievements and milestones, including the end of COVID-19 and monkey pox as global health emergencies.
But it was also a year to mourn; a year marred by conflict, disaster and preventable disease and death.
2023 was a reminder of why the work we do is so important, and why the world needs a strong, sustainable, effective, efficient and empowered WHO.
And that is why we’re here this week.
As always, we have a full agenda, reflecting the huge range of work of our Organization.
I look forward to our discussion on GPW14 today, as we move towards its finalization and, hopefully, its adoption at the World Health Assembly.
As I do every year, I would like to take a few moments to give you a snapshot of some of last year’s key achievements, based on the framework of the “Five Ps”.
First, promoting health.
This includes our shared work to address the root causes of disease, in our changing climate, the conditions in which people live and work, and in the products they consume.
Tobacco use is declining in 150 countries, and there are now 19 million fewer smokers globally than there were two years ago.
On trans fats, bans or limits were passed in six countries last year, and became effective in a further seven.
And every year, we support more and more countries to address child wasting and obesity;
To encourage breastfeeding;
To increase taxes on alcohol, tobacco or sugary drinks;
To regulate marketing of unhealthy products to children, including e-cigarettes;
To promote physical activity;
And to make our roads safer.
Now to the second of the “Five Ps”, to provide health, by supporting countries in their journey towards universal health coverage, based on strong primary health care.
This area includes our shared work on strengthening health systems, and the services they deliver for maternal and child health and communicable and noncommunicable diseases.
In the political declaration, countries made more than 50 commitments to progressively expand access to essential health services, and to improve financial protection.
Now to the third of the “Five Ps”: protecting health.
2023 was a significant year in our work to prepare for, prevent and respond to health emergencies.
After almost three and a half years, in May I declared an end to both COVID-19 and monkey pox as global health emergencies, although both remain global threats.
More than two-thirds of the global population has now received a complete primary series of a COVID-19 vaccine.

The Pandemic Fund 팬데믹 기금
We continue to urge all Member States to maintain and reinforce the capacities they built during the pandemic as part of their commitment to preparedness for future emergencies.
Meanwhile, there were many other crises demanding our attention last year.
In all, WHO responded to 65 graded emergencies, from earthquakes in Türkiye and Syria, to conflict and insecurity in the Democratic Republic of the Congo, Ethiopia, Haiti, Myanmar, Sudan, Ukraine, and of course the occupied Palestinian territory, especially Gaza.
And emergency medical teams played a vital role in our response to 19 emergencies in 18 countries - so just giving the list because if I tried to explain what we did in all of these emergencies in these countries it would take all day. But at least you know what we have been doing from various reports.
While our work responding to emergencies often makes the headlines, our work to prevent and prepare for emergencies is less visible, but equally important.
The WHO’s framework provides a roadmap for the governance, systems, tools, workforce and financing needed to strengthen national, regional and global health security.
We have many ongoing initiatives in this area, as you know.
For example, the Pandemic Fund is up and running, and has made its first round of disbursements, of $ 338 million to 37 countries.
The second round is now underway, with funding of $ 500 million.
At the political level, world leaders agreed a strong political declaration.
But I must say I am gravely concerned that Member States may not meet that commitment.
Time is very short, and there are several outstanding issues that remain to be resolved.
Everyone will have to give something, or no one will get anything.
I urge all Member States to work with urgency and purpose to reach consensus on a strong agreement that will help to protect our children and grandchildren from future pandemics.
Excellencies, dear colleagues and friends,
To promote, provide and protect health is the overarching goal of GPW14.
Those three core priorities are supported by the fourth and fifth Ps – to power and perform for health.
Powering health is about harnessing the power of science, research, digital technologies, data and communications to support countries most effectively to accelerate towards the SDG targets.
The fifth and final P, performing for health, means our continued work to build a stronger WHO.
This builds on the seven pillars of the transformation, which we began six-and-a-half years ago, to strengthen our strategy, operating model, processes, partnerships, financing, workforce and culture.
We are continuing to make progress in each of these areas.
Excellencies, dear colleagues and friends,
2024 will be a defining year for global health, and for our WHO.
It is a year of opportunity.
This year, you have the opportunity to shape the world’s global health strategy for the next four years, in the GPW14.
I urge you to seize these opportunities. They do not come very often.
We are all here because we want to make a difference, because we believe in the mission and vision of the World Health Organization.
This year is our opportunity to make that difference, and to make the world a healthier, safer, fairer place - and especially, a more peaceful place.

2020년 초에 시작된 코로나 팬데믹이 우리 인류에게 얼마나 큰 재앙이었는지 모두 기억하시나요? WHO는 2020년 1월 30일 팬데믹을 선언했고 인류는 신종 바이러스의 출연과 전염병의 공포에 떨어야 했습니다. 그리고 무려 3년이 지난 2023년 5월 5일, WHO가 팬데믹의 종식을 선언했습니다. 전 세계인이 긴 고통의 터널을 지나 신종 바이러스를 극복하기까지 지대한 역할을 했던 건 과연 무엇이었을까요?
이번 노벨 생리 의학상 수상자들의 연구가 밑바탕이 된mRNA 기반 코로나 백신입니다. 우리 인류는 그동안 전염병이 창궐할 때 백신을 통해 바이러스의 감염에 대항하고 집단 면역을 형성해서 집단 감염에 대항해 왔습니다. 그렇다면 코로나 팬데믹 이전에는 어떤 종류의 백신이 이용되고 있었을까요? 전통적인 백신은 바이러스를 약화시키거나 비활성화 시킨 다음, 이를 인체의 주사해 바이러스에 대항할 항체를 생산하도록 면역 활성을 유도합니다.
그러나 백신을 만들기 위해서는 많은 비용과 긴 시간이 필요합니다. 코로나 19와 같이 빠르게 번지는 신종 바이러스에 신속히 대항하기에는 역부족이라 우리 인류는 속수 무책 당하고만 있었습니다. 그런데 바로 이때 신종 바이러스에 대항할 새로운 무기, mRNA 기반 코로나 19 백신이 등장했습니다. 코로나 19때문에 mRNA 백신 기술이 갑자기 등장한 것처럼 보일 수 있지만 사실 mRNA 기술의 역사는 매우 깁니다. mRNA 백신에서 가장 중요한 요소인 mRNA는 1961년 네이처에 보고된 이후 현재까지 60년 이상 활발히 연구가 이어지고 있습니다.
1990년대 들어서는 우리의 면역 시스템이 외부에서 들어온 mRNA를 거부하고 염증 반응을 유도한다는 사실을 알게 되었고, 이런 문제 때문에 대부분의 연구자들은 mRNA를 치료제로 이용하거나 백신을 만들 수 있다는 희망을 접었고 그 후로 20년 이상 이 분야 연구는 불모지가 되어 버렸습니다.
그러나 포기하지 않고 mRNA 연구에 매진한 과학자가 있었는데요, 그분은 바로 이번 노벨 생리 의학상 수상자인 카탈린 카리코 박사님입니다. 카리코 박사님은 30년 이상 mRNA 연구에 매달렸지만 연구 과정이 순탄치는 않았다고 합니다.
그러다가 우연히 같은 학교인 펜실베니아 대학의 드루 와이즈먼 박사님을 만나게 됩니다. 커리코 박사님은 오랜 기간 RNA를 연구하던 분이고 드루 와이즈먼 박사님은 면역학자로써 환상의 팀이라고 할 수 있습니다. 두 분의 연구결과는 2005년에 Immunity라는 저널에 발표되었습니다.
이후 커리코 박사님은 2013년 바이오앤텍에 이직해 코로나 mRNA 백신 디자인에 중요한 역할을 하셨습니다. 이후 바이오엔텍은 글로벌 제약회사인 화이자와 함께 코로나 백신을 성공적으로 개발해냈습니다.
여기서 잊지 말아야 할 것은 기초 생명과학이 얼마나 큰 힘이 있는지, 의학과 만났을 때 얼마나 큰 파급력이 있는지입니다. 현재 mRNA 백신 기술은 코로나와 같은 감염병 외에도 다양한 질환에 이용되도록 개발되고 있습니다.

최근 대학에 불고 있는 의대 쏠림현상 왜 그런 걸까요? 취업문이 점점 좁아지는 상황에서 의대 입학은 그나마 마지막 남은 개천에서 용 나는 곳이기 때문입니다. 실제 의대만 졸업하면 웬만한 기업의원 부럽지 않은 고소득이 보장됩니다. 2020년 기준으로 의사 평균 연봉은2억 3천만원이었고 개원의의 경우 평균 3억원에 육박했습니다.
그럼 의사만 되면 끝일까요? 아닙니다. 의사도 다시 전공별로 급이 나눠집니다. 위험하고 힘들지만 돈벌이는 잘 안되는 과와 여유 있게 진료하면서도 돈이 되는 과로 나눠집니다. 아시다시피 성형외과와 피부과 경쟁률은 말 그대로 박이 터지는 반면 소아청소년과 경쟁률은 바닥으로 수직 낙하하고 있습니다. 이밖에도 흉부외과 산부인과 등 이른바 생명과 직결된 필수 과목을 중심으로 정원 미달 현상이 심해지고 있습니다. 많이들 들어보셨을 의사 부족 현상이 여기서 나오는 겁니다.
더 큰 문제는 지역별 편차입니다. 사실 기피 과목이라고 하더라도 아직 의사가 아예 없는 것은 아닙니다. 문제는 그나마 있어도 수도권에 집중되어 있다는 겁니다. 비수도권에는 의사가 많지 않다보니 수술을 받기 위해서는 큰 도시로 원정에 나서야 합니다. 긴급하게 수술이 필요한 위급한 상황에서는 시간을 단축하는 게 무척이나 중요하지만 원정의료에서는 이 골든타임을 놓치기 십상입니다.
돈이 되고 사람들이 몰리는 곳에서만 병원들이 쏠리다 보니 정작 진료를 필요로 하는 사람들이 제때 진료를 못 받는 겁니다. 의사 수 부족도 한몫하고 있습니다. 우리나라에서 환자를 직접 진료하는 의사는 인구천명당 2.5명입니다. OECD 국가 가운데 우리나라보다 의사수가 적은 곳은 멕시코 한 곳 밖에 없습니다.
정부는 이런 문제를 해결하기 위해 의사 수를 늘리면서 공공의료와 지역의료를 활성화하기 위한 해법으로 공공의대를 꼽고 있습니다. 현재 의대에 입학 정원은 3,058명입니다. 이 정원은 2006년부터 18년째 그대로입니다. 정부는 공공의대를 만들어 정원을 늘리면서 지역에서 일할 의사를 미리 정하는 방식으로 추진하고 있습니다. 전액장학금을 주는 대신 10년 동안은 특정 지역에서 일하는 방식입니다. 방향은 맞지만 의무적으로 근무하는 것만으로는 부족합니다. 의사를 늘려도 의무기간이 끝나면 결국 수도권에 있는 피부과와 성형외과로 쏠리는 것을 막을 수 없기 때문이죠.
또한, 의사협회의 반대는 넘어야 할 산입니다. 이전 정부에서도 10년 동안 4,000명의 의사를 추가 양성하는 방안을 추진했지만 파업을 불사한 의사 단체 반발에 논의가 중단된 바 있습니다. 저출산 고령화로 인구가 줄어드는 상황에서 의사 수를 더 늘리는 것은 불필요한 경쟁을 부추긴다는 논리입니다.
먹고 사는 문제만큼 중요한 의료 문제는 지역 소멸과 저출산, 고령화 문제와도 연계가 되어 있습니다. 당장도 늦은 감이 있지만 하루 빨리 의료 공백과 지역 격차를 해결할 수 있는 방안이 나오길 기대합니다.
